Study On The Clinical Efficacy Of Qingjin Huatan Decoction In Treating AECOPD Phlegm Heat And Influence On Correlated Inflammatory Cytokine

Objective:To explore the effect of Chinese Decoction for Qingjinhuatan on host inflammation in patients with chronic obstructive pulmonary disease(COPD)during acute episode period by lung phlegm,the level of correlated inflammatory cytokine,provide the new theory basis for treatment of AECOPD with traditional Chinese medicine.Methods: 60 patients with inflammation of COPD during acute episode period by lung phlegm and heat constraint pattern between 50 to 70 years old were collected into the present study.They were randomly divided into the treatment and control group,with 25 patients in each group.In the clinical trial,patients in two groups were treated by different measures.For patients in the control group,routine treatment by basic medicine was applied.For patients in the treatment group,the Chinese Decoction for Qingjinhuatan by oral intake was supplied in addition to the basic medicine.The treatment course for each group lasted for 14 days.Furthermore,we statistically analyzed the differences in Clinical symptom and sign,oxygen,spasmolysis,phlegm,time of the antiinfection,levels of WBC,N%,CRP,PCT,IL-6,IL-8 in blood before and after treatmrat in two groups.We evaluated the potential effect of the Chinese Decoction for Qingjinhuatan on inflammation in patients with COPD.Results:(1)The effective rate of patients between two groups wascompared after treatment.In the treatment group,total effective rate was83.3%,which,was higher than that in the control group(70%),the treatment group in the TCM efficacy evaluation,cough,sputum volume,wheezing,chest tightness and other symptoms improved better than the control group(P<0.05);(2)The comparison of pulmonary function,blood gas analysis,the time of use antibiotics in before and after treatmrat of two groups patients.There were no significant differences in before treatmrat of two groups(P>0.05),There were no significant differences in before treatmrat of two groups.the FEV1%pred(%),FEV1/FVC%,PO2,pCO2 and antimicrobial use time have a significant difference with the control group(P<0.05).the treatment group have better then control group in improve ventilate,oxygen,shorten use of antimicrobial;(3)Both WBC,neutrophil,CRP and PCT levels in peripheral blood of two groups were decreased,and changes in these parameters in the treatment group were significantly than those in the control group.The differences in two groups were statistically significant(P<0.05);(4)There were no significant differences in IL-6,IL-8levels in peripheral blood between two groups prior to treatment.After treatment,IL-6,IL-8levels in peripheral blood of patients in the treatment group were lower than those in controls.The comparison between two groups showed statistically significant difference(P<0.05).Conclusion:On the basis of conventional therapy of western medicine combined with traditional chinese medicine on,clearing heat and resolving phlegm and activating blood circulation method in improving the clinical symptoms,lung function,hypoxia,hypercapnia,hemorheology,infection control effect than the conventional western medicine has certain advantages.which might be related to the inhibitory effect on the host inflammatory respons.
