ObjectiveBy observing the effects of the method of clearing heat and resolving phlegm on various indicators before and after the treatment of patients with chronic obstructive pulmonary disease with acute exacerbation of phlegm-heat stagnation syndrome,comprehensively evaluate its clinical efficacy and safety,and provide clinics for the treatment of acute exacerbations of chronic obstructive pulmonary disease with traditional Chinese medicine reference.MethodsAdopting a prospective randomized clinical control design plan,AECOPD patients with phlegm-heat obstructing lung syndrome who meet the inclusion criteria are randomly divided into a control group and a treatment group through a random number table.The control group is given basic western medicine treatment,and the treatment group is added to the control group.Use traditional Chinese medicine Qingjin Huatan Decoction for 10 days.Observe the changes of TCM syndrome score,expectorant quantitative score,CAT score,mMRC score,peripheral blood white blood cell count,neutrophil ratio,CRP,IL-6,oxygen partial pressure,carbon dioxide partial pressure before and after treatment,and comprehensively evaluate the curative effect.Results1.Baseline comparison:A total of 60 clinical cases were included,30 in the control group and 30 in the experimental group.There was no statistically significant difference between the two groups in terms of gender,age,pulmonary function classification,and smoking history(P>0.05),and they were comparable.2.Clinical efficacy:the total effective rate of the control group was 86.67%,and the total effective rate of the treatment group was 93.33%,the difference was statistically significant(P<0.05).There was a statistically significant difference in the total score of TCM syndromes between the two groups before and after treatment(P<0.05);compared with the control group,the difference in the total score of syndromes in the treatment group after treatment was statistically significant(P<0.05).3.Symptoms efficacy:In terms of main symptoms,the main symptoms of cough,expectoration,shortness of breath,and rales were improved after treatment in the two groups,and the difference was statistically significant(P<0.05).Compared with the control group,the treatment group improved significantly in symptoms such as expectoration and shortness of breath.In terms of secondary symptoms,irritability,dry mouth,stool and other secondary symptoms of the two groups improved after treatment,and the difference was statistically significant(P<0.05).Both groups showed no significant improvement in abdominal distension(P>0.05);compared with the control group Compared with the treatment group,the symptoms of dry mouth and urination were significantly improved,and the difference was statistically significant(P<0.05).The CAT scores of the two groups decreased after treatment,and the difference was statistically significant(P<0.05);compared with the control group,the CAT scores of the treatment group decreased after treatment,and the difference was statistically significant(P<0.05).The mMRC scores of the two groups decreased after treatment,and the difference was statistically significant(P<0.05);compared with the control group,the mMRC scores of the treatment group did not decrease after treatment,and the difference was not statistically significant(P>0.05).4.Expectorant effect:the effective rate of the control group after treatment was 26.67%,and the effective rate was 66.67%;the effective rate of the expectorant effect of the treatment group was 50%,and the effective rate was 46.67%,the difference was statistically significant(P<0.05);The quantitative expectorant scores of the two groups decreased after treatment,and the difference was statistically significant(P<0.05);compared with the control group,the quantitative expectorant scores of the treatment group decreased significantly,and the difference was statistically significant(P<0.05).5.Inflammation indicators:The levels of WBC,NEUT%,CRP and IL-6 before and after treatment in the two groups were significantly different(P<0.05).Compared with the control group,the treatment group CRP and IL-6 decreased,the difference was statistically significant(P<0.05);WBC,NEUT%did not decrease,the difference was not statistically significant(P>0.05).6.Blood gas analysis:The PaO2 and PaCO2 levels of the two groups before and after treatment had statistically significant differences(P<0.05).Compared with the control group,PaO2 and PaCO2 in the treatment group improved,and the difference was statistically significant(P<0.05).ConclusionThe addition of Qingjin Huatan Decoction to the basic treatment of Western medicine can improve clinical efficacy,relieve symptoms,reduce inflammation,improve respiratory function,and reduce airway mucus hypersecretion.No adverse reactions have been reported in clinical application,and the safety is relatively high.The method of clearing heat and resolving phlegm in the treatment of acute exacerbations of chronic obstructive pulmonary disease with phlegm-heat stagnation syndrome has shown high clinical value and is worthy of further clinical promotion.
