Clinical Observation On Treatment Of Senile Chronic Obstructive Pulmonary Disease With Acute Exacerbation Of Phlegm Heat And Blood Stasis Syndrome By Qingjin Zhuyu Huatan Decoction In The Treatment

Objective:The clinical therapeutic action and security were observed by using Qingjin Zhuyu Huatan Decoction to treat senile patients with phlegm-heat blood stasis syndrome during acute exacerbation of chronic obstructive pulmonary disease.Methods:Sixty-six elderly inpatients were selected whom met the inclusion condition of AECOPD syndrome of phlegm heat blood stasis from December 2019 to January 2021.According to the randomized control principle,33 cases who were divided into the control group would treated with standard medicine.The other group,33 cases were additionally given Qingjin Zhuyu Huatan Decoction（QZHD）for per os after meal,one 150ml bag and three times a day.The observation lasted for 10 days in both groups.The data that including symptom score,coagulation function,blood gas marks,lung function,inflammation marks,CAT score and other outcome indicatrix of the two groups of patients was recorded.The data was able to judge the therapeutic action of QZHD after analysised by SPSS20.0.Meanwhile,the changes of security indicators（blood routine,electrocardiogram,routine defecation,liver function,electrolyte,kidney function,etc.）were monitored to judge the security of the treatment.Results:（1）The scores of TCM single symptom and syndrome:The primary and secondary symptoms of patients in both groups were obviously improved（P<0.05）after treatment.The improvement in the thirst score and the total score of syndrome in the treatment group was remarkable better than the control group（P<0.01）,and the ameliorationof cough,yellow phlegm and dry and difficult stool in the treatment group was exceeding than the control group（P<0.05）.（2）Laboratory indicators:The treatment group was more remarkable in reducing inflammatory markers（CRP,PCT）（P<0.01）and（WBC,NEUT%）（P<0.05）.The coagulation indicators（FIB,D-D）（P<0.01）which had brought down apparently in the treatment group.As to the lung function,the treatment group also had an advantage in bringing down Pa CO₂ and enhancing FEV1%pred（P<0.05）.（3）Quality of life:Both groups had a inferior CAT scores after treatment,but the therapeutic group was remarkable better in cutting down CAT scores（P<0.01）.（4）Safety indicators:no adverse reactions occurred in all cases during the test,and no clinically significant deterioration was observed in security indicators.CONCLUSIONS:Qingjin zhuyu huatan decoction for elderly AECOPD（phlegm heat and blood stasis syndrome）can notably improve patients’hypercoagulability and living quality,ameliorate the carbon dioxide accumulation and pulmonary ventilation,moderate cough or other symptoms.Meanwhile,it has a safety coefficient..
