Cardiovascular disease, diabetes and other chronic non-communicable diseases which are closely related to people life style, have become the main killer of health and life. Chronic diseases have the characteristic of long course, complex cause, relatively serious social harm, involving multiple organ dysfunction, etc. The prevalence of chronic diseases is rising and the risk factors of chronic disease are popular in our country. The rapid aging population and the increasing of elderly population make the prevention and control work of chronic disease more urgent and difficult. At present, the requirement of management work to chronic diseases is increasingly urgent.Nowadays, the symptoms treatment centered medical model is transferring to a4P model, which refers to Preventive, Predictive, Personalized and Participatory. The medical equipment is towards the trend of digitization, integration, miniaturization, familial and portable. At the same time, smart mobile devices are more extensive. The mobile health based on the mobile devices is becoming more and more favorable. Mobile health has the characteristics of convenient, flexible and it greatly improve the accessibility of medical service. It is a practical and effective method to reduce the pressure of medical provides, in the condition of increasing pressure of current medical industry and the relative lack of medical resources.This paper, base on the analysis and research of mobile health in chronic disease health monitoring and management both at home and abroad, put forward the management and monitoring system for chronic disease signs mainly for elderly patients, which combined with chronic diseases in our country and the development trend of mobile medical. It has the characteristics of informatization, intellectualization, networked. It can optimize allocation of medical resources and arouse the enthusiasm of individual for disease management.This research comes from the projects of Guangdong, Hong Kong and Macao in key areas "Research and Application of key Technology of the Elderly health and Chronic Diseases Medical Monitoring and Management Based on the Internet of Things (NO:2011A011304001)". This project aims to apply the Internet of things technology, wireless communication technology and cloud computing technology to make an assessment, monitoring and health consultation platform for the patients with chronic disease.The goal of this research is provide no restriction of guardianship services for elderly chronic patients and provide doctors timely and useful clinical information, to achieve the purpose of supervision and prevention, eventually reduce the medical cost, to alleviate the pressure of the hospital and mitigation high cost of doctor. The research and implementation of the system has the meaning of:(1) Solve the problem of Long-term health tracking for chronic diseases.Due to the condition of hospital care, the economic condition of patients’ family, the constraints of personal time, there are the problem of insufficient care range chronic patients. The monitoring and management system for chronic symptoms based on mobile intelligent terminal, which has health sensor as input devices, the APP as the front, with professional health management service institutions as a background, enables users to long-term health monitoring and management and solves the problem of long-term monitoring and expense medical.(2) To realize the automation, paperless and standardization of health indicators record and manage.Long-term health monitoring will accumulate a lot of medical data. However, effective recording and managing of the data provide high demands to patients’ health management capabilities. Even though patients can record the data, the present of development trend of the data is also a big problem. The realize of the automation and paperless record of health indicators, not only improve the usability, convenience, also can improve the accuracy of the data records and make patients and physicians understanding of patients condition for a long time possible, so physicians can give personalized health care.(3) Improving people’s awareness about disease management.Chronic diseases have the characteristic of long course, complex cause, relatively serious social harm, involving multiple organ dysfunction, etc. However, the understanding of the whole society about chronic diseases is insufficient and individual’s lack of self health management consciousness. This system prompts warning for user’s signs in real time. Through the use of mobile intelligent signs of chronic diseases monitoring management service system can prompt the health management consciousness(4) Implement storage and reuse of monitoring signs data.Long term monitoring health data, not only have great help for patient to understand the development of disease, but could help doctors to make more accurate diagnosis when the patient in hospital, it also can help disease researchers do a lot of data analysis and mining work. The main research work of this research includes several aspects:(1) Research of Bluetooth interface and protocol of parse signs data.This thesis researched data transmission interface between mobile terminal and signs monitoring equipment, the wireless data transmission technology. Realized Bluetooth connection, this is the basis of the data uploaded to the intelligent terminal. At the same time, in order to achieve early warning and remind purposes, we use data protocol provided by the manufacturer of monitoring equipment complete the analytical work of packets and use of programming tools to display the data on screen of intelligent terminal.(2) Alarming model for abnormal data of personal health monitoring signs.The occurrence and development of chronic disease is associated with a variety of factors. However, patients often lack professional medical knowledge and understand little of medical background of health data. For help patients to dig deeper into the clinical significance of the signs and achieve real time alarm, this paper combines medical knowledge and uses a simple prompt identification interface to help patients understand their own disease progression timely.(3) Automatic assessment of signs and intervention model research.In order to make user understand the status of the disease, this research does statistical analysis for the monitoring signs data based on the stage that the user set up, and the statistical analysis abnormal value of the monitoring data.The realization of this Monitoring and Management System for Chronic Disease Signs has some innovations:(1) Put forward the monitoring scheme of health signs for patients with chronic diseases and solve the problem of long term health tracking.(2) Researched and implied the personalized signs warning function, convenient users understanding of their own health status and adjusting the diet, sleeping habits. (3) Achieved the automation, paperless, standardization of signs recording, solve the problem of management and utilization of medical data in low efficiency way.This thesis divided into six chapters:The first chapter introduces the research background and research meaning and the application of mobile health in health management aspects both at home and abroad and summarizes the research content and organizational structure of the paper. The second chapter researched the requirements, feasibility and the overall architecture, process design. The third chapter researched the key technology and model of system, including data interface protocol and Bluetooth data transmission technology, automatic assessment of signs and intervention model etc. The fourth chapter introduces the design and implementation of client terminal. Specific data collection, storage, display and uploaded to the server process. The fifth chapter is the system theory knowledge, the system of testing results. The sixth chapter is summarized and prospected. It includes research summary and outlook for mobile medical industry.At present the mobile intelligent monitoring and management service system for chronic diseases signs has been basically realize its functions. Test results show that the overall system design is reasonable and it can solve the problem of chronic long-term health track. It can achieve signs data record and management automation, paperless, standardized and the functions of signs warning prompt and automatical evaluation of signs.
