Timing Of Scientific Fluid Therapy And Transportation Influencrd On The Prognosis Of Group Burnt Patients

BackgroundBurn is a common trauma in the daily life and work,which threatens people’s physical and mental health seriously.After 58 years of rapid development,the level of burn treatment have made a great improvement and reached the top in the world.Nowadays the cure rate of patients suffering from medium and small area burns has reached more than 97%.The difficulty of treatment of burns focuses on the patients suffering from extensive burns.With the development of economy, the change of social structure and situations,a series of unexpected group incidents have already happened in the recent years,which had led to a large number of burn patients. Group burnt incidents have the features of plentiful wounded persons,complicate conditions and heavy tasks of curing and treating the patients, which affects the further development of burn therapy.There are a series of researchs on curing and treating the patients of group burnt incidents in many areas in China in the recent years.The pre- hospital emergency medical service in group burnt incidents had become the focus of burn medical feild.ObjectivesCollect the data of patients in group burnt incidents,who are treated in the First Affiliated hospital of Guangxi Medical Univesity between January 1,2013 and December 31,2015.Analyze it and find the relationship between complications,prognosis and the timing of scientific fluid therapy and transportation,which can provides reference for the treatment of group burnt patients.MethodsCollect the data of patients in group burnt incidents, who are treated in the First Affiliated hospital of Guangxi Medical Univesity between January 1,2013 and December 31,2015. The data were canceled who automatic discharged from the hospital,had used steroid in a long time and had got dyscrasia,diabetes, inborn immune deficiency, which would influence on normal function of immune system. Count up the data including the number of patients and incidents,gender,age,injured places,pathogeny,TBSA,combined injury,inhalation injury,timing of scientific fluid therapy and transportation,septicopyemia,MODS and prognosis.Assign them to earlier scientific fluid therapy group (≤3h) and later scientific fluid therapy group (>3h) according to the time of scientific fluid therapy, compare the differences of septicopyemia, MODS and prognosis between two groups.Assign them to reasonable transportation group and unreasonable transportation group according to the time of transportation,compare the differences of septicopyemia, MODS and prognosis between two groups.In order to explore the timing of scientific fluid therapy and transportation influenced on the prognosis of group burnt patients.Results1. Septicopyemia, MODS and prognosis present obvious statistical differences (p<0.01) between earlier scientific fluid therapy group (<3h) and later scientific fluid therapy group (>3h)2.Septicopyemia,MODS and prognosis present obvious statistical differences(p < 0.01) between reasonable transportation group and unreasonable transportation group.Conclusions1. Group burnt patients are likely with inhalation injury because of relatively closed environments.Right Self-aid may help lessen the severity of inhalation injury obviously.2. Earlier scientific fluid therapy can restore intravascular volume,ameliorate circulatory disorders,immprove the perfusion of tissue.3. Reasonable schedule of transportation can ensure their arrival to hospital radpidly and saftely and make sure that they will get the high level of treatment in the hospital as soon as possible.It plays a positive role to their prognosis.
