ObjectiveEvaluate the curative effect of electroacupuncture in the treatment on neck pain of cervical spondylosis through a contrastive study of EA and sham EA.Methods74 cases recruited eligibly from outpatient service of Guangdong Province Hospital of TCM is randomly divided into two group which includes the electroacupuncture group and the shame EA group. TheSl15, BU14, SJ3 and Jiaji, which is found from cervical positive reaction place, are selected for the acupoints of both group, while the operating mode accords to part 11 about EA in the national standard of the People’s Republic of China(GB/T 21709.11-2009). The difference between two group is the special needle called Streitberger Needle and produced by Germany, whose blunt needle could rebound to its stents when it contact the skin. Every patient is ordered to be treated three times a week and finish 10 times, or,8 times at lease. We regard MPQ, NPQ, MOS-SF36 and Overall Evaluation as obvervational indexs of our study and record the situation before the treatment, at the end of the treatment and lmonth of the end. Adverse events or severe adverse events were to be recorded at any time during the treatment.Statistical method:Data of this research was calculated and analyzed by the software SPSS19.0. Measurement data was demonstrated in the means of (x ±S), while enumeration data was demonstrated in the means of percentage. The paired t-test or t’Test was applied in the comparison within groups, while the variance analysis and pairwise comparison was applied in the comparison between groups. Repetitive measurement deviation analysis was applied in comparion of the measurement data at all time points observed. The rank-sum test was applied when there was a heterogeneity of variance. The chi-square test and fisher’s exact test were applied in the comparison of total effective rate between groups. Test size:α=0.05, two-sided test.Results1. Comparison of the baseline prior treatment:There is of no statistical significance between the age, Gender, educational level, course of the disease, treatment history, and syndrome types of TCM, neither do the scores of MPQ, NPQ and SF-36 before the treatment between two groups. Therefore, two groups are comparable.2. The Neck Pain Questionnaire Percentage:At different observation points including "before treatment", "at the end of the treatment" and "1 month after the end" the means of the EA group and shame EA group severally were 29.73%+± 12.81%,23.27%± 13.18%,25.08%± 16.27%and 28.70%± 14.13%,26.39% ±14.46%,33.61%±15.34%.Intra-group comparison:In the aspect of EA group, the means of "before treatment" are greater than the other two observation points’, which there are significant differences. But there was of no statistical significance between "at the end of the treatment" and "1 month after the end". In the aspect of shame EA group, the means of "1 month after the end" are greater than the other two observation points’, which there are significant differences. But there was of no statistical significance between "before treatment" and "at the end of the treatment" Comparison among groups:There was no statistical significance before and at the end of treatment about the difference between groups, but not for the one 1 month after treatment, at which the percentage of the EA group less than shame EA group.3. The Mcgill Pain Questionnaire:At different observation points including "before treatment", "at the end of the treatment" and "1 month after the end" the means of the EA group and shame EA group severally were 115.84+ 7.559、8.89±6.095、9.16±6.862 and 13.83±6.379、11.23±8.369、13.73±6.792. Intra-group comparison:In the aspect of EA group, the means of "before treatment" are greater than the other two observation points’, which there are significant differences. But there was no statistical significance between "at the end of the treatment" and "1 month after the end". In the aspect of shame EA group, the means of "at the end of the treatment" less than the other two observation points’, which there are significant differences. But there was of no statistical significance between "before treatment" and "1 month after treatment"Comparison among groups:There was no statistical significance before and at the end of treatment about the difference between groups, but not for the one 1 month after treatment, at which the scores of the EA group less than shame EA group.4. The SF-36 Questionnaire(1) Total ScoreAt different observation points including "before treatment", "at the end of the treatment" and "1 month after the end" the means of the EA group and shame EA group severally were 110.80±29.1、129.51±32.47、127.31±39.65 and 121.50±33.88、129.41±35.67、124.91±33.76.Intra-group comparison:In the aspect of EA group, the means of "before treatment" are less than the other two observation points’, which there are significant differences. But there was no statistical significance between "at the end of the treatment" and "1 month after the end". In the aspect of shame EA group, the difference of means at different observation point are not significant differences.Comparison among groups:There was no statistical significance, the value of P is greater than 0.05.(2) Comparison of PCSAt different observation points including "before treatment", "at the end of the treatment" and "1 month after the end" the means of the EA group and shame EA group severally were 57.09±16.77、65.57±18.43、64.22±19.89 and 63.08±18.34、63.42±20.06、62.38±19.16.Intra-group comparison:In the aspect of EA group, the means of "before treatment" are less than the other two observation points’, which there are significant differences. But there was no statistical significance between "at the end of the treatment" and "1 month after the end". In the aspect of shame EA group, the difference of means at different observation point are not significant differences.Comparison among groups:There was no statistical significance, the value of P is greater than 0.05.(3) Comparison of MCSAt different observation points including "before treatment", "at the end of the treatment" and "1 month after the end" the means of the EA group and shame EA group severally were 51.06±6.67、60.66±16.68、59.82±18.23 and 55.85±18.50、63.03±18.27、59.93±17.80.Intra-group comparison:In the aspect of EA group, the means of "before treatment" are less than the other two observation points’, which there are significant differences. But there was no statistical significance between "at the end of the treatment" and "1 month after the end". In the aspect of shame EA group, the means at the end of treatment are greater than the one before treatment, whose differences are of statistically significant. But the differences of means between "before treatment" and "1 month after the end" are of no statistically significant, neither between "1 month after the end" and "at the end of the treatment"Comparison among groups:There is no statistical significance, the value of P is greater than 0.05.5. Effective RateAt the end of treatment, the improvement rate and the total effective rate of EA group partly are 24.30%and 48.60%. Those of sham EA are 16.67%and 33.33%. After rank sum test, the differences between two groups are of no statistically significant.1 month after the end, the improvement rate and the total effective rate of EA group partly are 18.90%and 43.20%. Those of sham EA are 0%and 10.00% (Z=-3.107,p=0.002)Conclusion1. Electroacupuncture and sham EA can alleviate cervical pain to a certain extent. The curative effect of electroacupuncture group is better than that of sham EA group after one month.2. In the improvement of NPQ, MPQ and physical health score, the electro acupuncture group was better than the sham EA group, and there was no significant difference between the electro acupuncture group and sham EA group in improving mental health.
