Intramedullary Nail With Steel Plate Treated With LISS Calf Lower Segment Fractures Of Clinical Observation

Objective: To observe the clinical effect of application of fracture with LISS plate in the treatment of leg lock intramedullary nail, to explore the lower middle leg fracture treatment options.Methods: retrospectively in our hospital from 2012 February to 2014 December in Department of orthopedics were treated 40 cases of fresh fracture of middle lower part of the leg by different surgical methods in the treatment of observation and evaluation after the recovery of limb function in Johner-Wruhs tibial shaft fracture evaluation standard as evaluation standard, two surgical methods of observation variables for the duration of operation, intraoperative the amount of bleeding, postoperative swelling time, wound healing time, hospitalization time. Included were 40 cases, interlocking intramedullary nail fixation group of 20 cases, 20 cases of LISS plate group. In 40 patients, 14 to 87 years old, the average age of 45.25 years old.Results: the 40 cases patients were followed up. The follow-up time was 3 to 8 months, average 5 months, failure of internal fixation and postoperative complications were not found in all the patients. At the time of the latest follow-up according to Johner-Wruhs score, interlocking intramedullary nail fixation group was excellent in 14 cases, good in 3 cases, 3 cases, the excellent and good rate reached 85%; the LISS plate group were excellent in 14 cases, good in 4 cases, 2 cases, excellent rate reached 80%, achieved a good therapeutic effects. Two groups of duration of operation is: the interlocking intramedullary nail group(76.95±5.925) minutes, LISS plate group(69.75±4.266) minutes, the results were tested by χ2, P < 0.05, there was statistical significance, LISS plate group and the operating time is less than the interlocking intramedullary nail group. The two group of intraoperative bleeding: interlocking intramedullary nail group(88.15±4.932) ml, LISS plate group(127.1±6.972) ml, the results were tested byχ2, P < 0.05, there was statistical significance in LISS plate group, intraoperative blood loss more than locked intramedullary nail group. Two groups of swelling time: intramedullary nail group(5.15±0.988) days, LISS plate group(8.05± 0.945) days, the results were tested by χ2, P < 0.05, there was statistical significance, LISS plate group postoperative swelling time than locked intramedullary nail group. Two groups of knife edge healing time was: interlocking intramedullary nail group(14.05 ± 0.826) days, LISS plate group(14 ±0.725) days, the results were tested by χ2, P > 0.05, there was no statistical significance. Time in the hospital for the two groups: interlocking intramedullary nail group(15.85±1.496) days, LISS plate group(15.95±0.808) days, the results were tested by χ2, P > 0.05, there was no statistical significance.Conclusion: the interlocking intramedullary nail and LISS plate in treating distal fracture of the leg can achieve good fixing effect, interlocking intramedullary nail group, amount of bleeding during the operation, wound healing time and postoperative swelling time than LISS Steel Group has an advantage, while LISS Steel Group in the operation time than with interlocking intramedullary nail group.
