Objective:To study the observation and comparison of the curative effect analysis of the middle and lower segment tibia fractures of MIPPO steel plate and intramedullary nail,to provide the reference for clinical treatment of the middle and lower segment tibia fracturesMethods :Eighty-three cases of tibial fracture patients in February 2010 to April 2013 from the first hospital affiliated to dalian medical university were retrospectively analyzed.In the 83 cases,interlocking intramedullary pin fixed group of 42 cases, MIPPO group 41 cases, two groups of patients were hospitalized and to be completed the operation of MIPPO and intramedullary nail.To observe:preoperative tissue recovery time,operativetime,intraoperative X-ray fluoroscopy time accumulated, blood loss,postoperative complications(swelling, necrosis, infection), fracture healing, AKS knee joint fuction score Kofoed ankle wruhs score.Results:In the perioperative related project,the differences of closed reduction and interlocking intramedullary nails and percutaneous minimally invasive plate project in statistics are preoperative time 、 operation time 、 cut complications and derma intraoperative X-ray fluoroscopy time accumulated,MIPPO steel plates preoperative tissue recovery time is longer than nails,MIPPO, on average, 6.3 ±1 days,nails on average,4.2±1 days,operation time, intramedullary nail on average about 80.5 ± 5 minutes, MIPPO, on average, 60.7±5 minutes, intramedullary nail on average about 80.5± 5 minutes, there are 3 cases of infection and skin necrosis in MIPPO and infection are all the soft tissue infection, without infection of bone, intramedullary nail infection and skin necrosis both are 1 case, MIPPO infection and skin necrosis rate is higher than the interlocking intramedullary nail,intraoperative X-ray time accumulated the intramedullary nail is longer, the average cumulative perspective statistics was 29.3 ±3 seconds, MIPPO steel average cumulative perspective time is 17.6 ±4 seconds,Bone healing rate which were observed showed no statistical differences,and functional score in 18 months, knee scores and ankle score showed no statistical differenceConclusion:Treatment of the middle and lower segment tibia fractures intramedullary pin and MIPPO steel plate are both effective methods, two kinds of minimally invasive surgery is worth the clinical application.Intramedullary nail should be recommended in the patients with bad soft tissue condition and wevere swelling, MIPPO steel plate should be recommended in the patients with severe fractures.
