Objective:To discuss the result of treating complicate tibiofibular fracture by percutaneous locking bridging-plate osteosynthesis and interlocking intramedullary nail, to provide the data for the clinical application.Methods:From 2000 to 2006, 41 patients who suffered from complicate tibiofibular fracture were divided into 2 groups(A and B) at random. Combining indirect redution , treated by percutaneous locking bridging-plate osteosynthesis and interlocking intramedullary nail. Compare them with surgery time,intraoperation beeding,fluoroscopical time,time of well-subsidese ,inhospital time, union rate of 3 and 6 months, complications rate ,postoperative therapeutic effect.Results:There are not significant difference in surgery time, that was 45 to 135 minutes in group A(average 65.87±20.70min) and 50 to 165 minutes in group B (average 70.83±25.90min). There are significant difference in the intraoperation beeding,that was 50ml to 170ml (average 81.04±27.85ml) in group A, 60ml to 260ml (average 136.67±27.38ml) in group B. There are not significant difference in fluoroscopical time in operation, that was 1 to 10 minutes (5.57±2 .09min) in group A and 1 to 12 minutes (4.22±2.96min) in group B. The time of well-subsideise in group A (5 to 10 days,average 7.22±1.36days) is less than group B (6 to 14 days,average 8.67±1.75days), there are significant difference. The inhosoital time was16.61±2.04days in group A, was 16.83±2.92days in group B , There are not significant differences. In group A fracture union rate of 3 months was 95.65%, in group B fracture union rate of 3 months was 66.7%, there are significant differences. In group A, fracture union rate of 6 months was 100%, in group B fracture union rate of 6 months was 94.44% , there are no significant differences. The rate of complications postoperative was 8.7% in group A, was 11.1% in group B, there are not significant difference. The postoperative therapeutic effect in group A and B is not significant differemce.Conclusion:Treatment of complicate tibiofibular fracture with the percutaneous locking bridging-plate osteosynthesis and interlocking intramedullary nail are both ideal therapies. It is obvious that the percutaneous locking bridging-plate osteosynthesis in the complicate tibiofibular fractures have many advantages.,it can reduce disturbing vascularization of borne fragment , decrease intraoperation beeding and the time of well-subsideise, accelerate bone healing.
