Clinical Application Study Of Laparoscopic Inguinal Hernia Repair Using A Totally Extraperitoneal Prosthetic Approach

OBJECTIVE: To investigate the clinical safety and efficacy of laparoscopic inguinalhernia repair using a totally extraperitoneal prosthetic approach.METHODS: A total of78adult males with inguinal hernia was enrolled into thisclinical trial during the period from July2012to June2013at The Second AffiliatedHospital of SooChow University and Suzhou Xiangcheng People Hospital. Patients weregrouped into two arms treated with laparoscopic repair (n=42) and open tension-free meshrepair (n=36), respectively. Comparison of laparoscopic repair and open tension-free meshrepair was performed in this non-randomized comparative study according to theevaluation criteria of clinical efficacy, safety,, health economics, and patient’s lifesatisfaction.RESULTS: For clinical efficacy, when compared to the open tension-free mesh repair,patients benefited from laparoscopic repair in bleeding, pain score, hospitalization, andrecovery for normal activities but except operation time. For clinical safety, whencompared to the open tension-free mesh repair, the laparoscopic repair resulted in lowerrelapse rate and comparative complication rate but without observed significant differences(P>0.05). For health economics, when compared to the open tension-free mesh repair, thelaparoscopic repair achieved comparative economic benefit but gained more patients’satisfaction.CONCLUSION: When compared to the open tension-free mesh repair, thelaparoscopic repair of inguinal hernias using a totally extraperitoneal prosthetic approachwas conclude as a safety and efficient approach for the treatment of adult inguinal hernias.
