A Clinical Comparative Study Of Laparoscopic Inguinal Hernia Repair And Open Tension-free Hernia Repair

ObjectiveBy comparing of the clinical effects and characteristics of laparoscopic inguinal hernia repair(TAPP)and open tension-free hernia repair(Lichtenstein),we select a better surgical scheme for the treatment of inguinal hernia MethodThe clinical data of 151 cases of adult inguinal hernia from February 2010 to February 2015 were retrospectively analyzed,random grouping and including 68 cases of TAPP and 83 cases of Lichtenstein.The operative time,intraoperative blood loss,postoperative hospitalization days,total hospitalization expenses,anus exhaust and feeding time,out of bed activity time,postoperative pain VAS score and postoperative complications of two operative methods were compared.ResultsThe two groups of patients were successfully completed and followed up for 24 months.Two groups of postoperative hospitalization time,anal exhaust and eating time,postoperative Pain VAS score one and three months and postoperative complications(postoperative incision infection,intra-abdominal hemorrhage,hematoma,seroma or hematoma of scrotum or effusion and recurrence)were no significant difference.Postoperative hospitalization time in group TAPP and group Lichtenstein was(8.04±1.58 vs.8.09±1.71 P=0.857)d,anal exhaust and eating time was(1.68±0.60 vs.1.65±0.58 P=0.746)d,postoperative Pain VAS score in one month was(1.19±0.76 vs.1.26±0.78 P=0.607)scores,postoperative pain score of VAS in three months was(0.64±0.59 vs.0.69±0.65 P=0.632)scores,postoperative complications was(4.11%vs.5.13% P=1.000).Lichtenstein group had one patient of postoperative recurrence,TAPP group had no recurrence.The amount of operation time in group TAPP and Lichtenstein group was(75.79±11.79 vs.44.22±6.54 P=0.000)min;The amount of bleeding was(9.18±4.25 vs.11.41±3.86 P=0.001)ml;The total hospitalization expenses were(17193.27±1361.09 vs.7312.18±1359.96 P=0.000)yuan;Postoperative 24 h pain VAS score was(2.62±0.68 vs.3.38±0.76 P=0.000)scores,and the results of the two groups were statistically significant(P>0.05).ConclusionThe amount of bleeding during laparoscopic hernia repair of inguinal surgery is less,occult hernia can be treated simultaneously during the operation,postoperative 24 h pain VAS score was lower,the time to move down the bed is early and the recovery is fast.We think that the treatment of adult inguinal hernia should be the first choice of laparoscopic inguinal hernia repair.
