Therapeutic Effect Of Open Tension-free Hernia Repair And Laparoscopic Peritoneal Hernia Repair In The Treatment Of Inguinal Hernia

Objective: To evaluate the efficacy of open tension-free hernia repair and laparoscopic peritoneal hernia repair in the treatment of incarcerated inguinal hernia,and provide a reference for the clinical treatment of incarcerated inguinal hernia.Methods: Eighty patients with incarcerated inguinal hernia were enrolled in the study.All patients were admitted to the hospital from February 2016 to June 2018.They were divided into two groups.One group was the control group(40 cases)and the other group was the study group(40 cases).In the control group,inguinal hernia incision and open reduction and tension-free hernia repair were used.The study group was treated with inguinal hernia,laparoscopic reduction and peritoneal hernia repair(TAPP).The patients in the inguinal hernia were compared.The amount of bleeding,operation time,length of hospital stay,postoperative gastrointestinal function Recovery time,pain score before and after surgery,hospitalization costs,total effective rate of surgery,postoperative complications(incision infection,urinary retention,seroma,subcutaneous effusion,etc.)and postoperative recurrence rate.Results:(1)The amount of intraoperative blood loss.The intraoperative blood loss of 40 patients with incarcerated inguinal hernia was(4.93±1.34)ml,and the intraoperative blood loss of 40 patients with inguinal hernia in the control group was(10.28±1.63)ml.The intraoperative blood loss was lower,and the difference was statistically significant(P <0.05).(2)Operation time.The operation time of 40 patients with inguinal hernia in the study group was(79.63±10.09)min,and the operation time of 40 patients with incarcerated inguinal hernia in the control group was(58.78±9.54)min.Compared with the control group,the study group had longer operation time.The difference was statistically significant(P < 0.05).(3)Hospitalization time.The length of hospital stay in 40 patients with incarcerated inguinal hernia was(4.28±0.94)d,and that in 40 patients with inguinal hernia in the control group was(7.09±1.03)d.Compared with the control group,the study group had shorter hospital stay.The difference was statistically significant(P<0.05).(4)Postoperative gastrointestinal function recovery time: postoperative gastrointestinal function recovery time of 40 patients with inguinal incarcerated hernia in the study group(1.45±0.28)d,and postoperative gastrointestinal function recovery time of 40 patients with inguinal incarcerated hernia in the control group(2.51± 0.43)d;Compared with the control group,the postoperative recovery time of gastrointestinal function in the study group was shorter,and the difference was statistically significant(P < 0.05).(5)Pain score before and after surgery.There was no significant difference in preoperative pain score between the two groups of incarcerated inguinal hernia(P >0.05).Before operation,the postoperative pain scores of the inguinal hernia in the two groups were lower,and the difference was statistically significant(P < 0.05);Compared with the control group,the postoperative pain score was lower in the study group,and the difference was statistically significant(P <0.05).(6)Hospital costs.The hospitalization cost of 40 patients with inguinal hernia in the study group was(8176.63±648.29)yuan,and the hospitalization cost of 40 patients with inguinal hernia in the control group was(7052.94±618.92)yuan.Compared with the control group,the study group had higher hospitalization costs.The difference was statistically significant(P < 0.05).(7)The incidence of postoperative complications.The postoperative complication rate was 5.00%(2/40)in the study group and the invasive hernia of the inguinal hernia in the study group was27.50%(11/40).Compared with the control group,the incidence of postoperative complications was lower in the study group,and the difference was statistically significant(P <0.05).(8)Postoperative recurrence rate.There were no recurrence in 40 patients with incarcerated inguinal hernia in the study group,and the recurrence rate in 40 patients with inguinal hernia in the control group was 7.5%(3/40).Compared with the control group,the recurrence rate was lower in the study group.Statistical significance(P <0.05).Conclusion:(1)Laparoscopic transperitoneal hernia repair is more effective than open tension-free hernia repair in the treatment of inguinal hernia.It mainly covers the abdominal cavity of the patient during the operation and effectively prevents the necrosis of the intestine.(2)Compared with open tension-free hernia repair,laparoscopic transperitoneal hernia repair without tension in the treatment of inguinal hernia is less traumatic,less intraoperative blood loss,hospitalization time,postoperative gastrointestinal function recovery time Shorter,postoperative complication rate and recurrence rate are lower,but the operation time is longer and the hospitalization cost is higher.(3)Laparoscopic transperitoneal hernia repair for inguinal hernia treatment can further improve the efficacy,reduce pain,and fewer surgical complications,low recurrence rate and high safety.
