BackgroundWith the process of nursing specialty, specialty nurses who are advanced practice nursing talents developed. At present, there is no unified evaluation index for the specialty nurse in china. As the procedure of hospital’s evaluation standards and qualification identification is different,the evaluation results comes out with larger difference.lt is not conducive to the specialty nurse play a role and scientific management.So, it is very necessary to establish a standardized unified nurse certification and evaluation index evaluation system.ObjectiveTo investigate the inclusion criteria and certification procedures of specialty nurse and build the evaluation index system for specialty nurse in line with China’s national situation.It can provide a reference for formulating the normative unified evaluation index, realize systematization and standardization of the certification and evaluation for specialty nurse.Methods1.Establish the research groupThis research group had 4 people, among them one with the title of Professor, one was a lecturer of nursing,the other two were Master graduate students.2.Literature researchConsulted literature related to the certification and evaluation for the specialty nurse in Wanfang,CNKI,and PubMed database.On the basis of these material,Analyze the status of specialty nurse at home and abroad,then get the frame structure of the index system.3.semi-structured interviewBy convenient sampling method,Conduct semi-structured interviews on the selection of experts, combined with the actual situation of nursing in china, following the scientific, brief, practical, operable principle built a initial framework for our certification and evaluation of Specialty Nurse.4.Delphi methodAccording to the preliminary construction index draft, we design expert consultation questionnaire. By convenient sampling method, selected 23 experts from 4 provinces Shanghai, Jiangsu, Hunan, Henan,who go in for nursing management, nursing education, clinical care, clinical care and related fields take part in the research and give suggestions for each index and according to Likert5-scale method for evaluation and selection. The survey end until the experts’opinions unified.The inclusion criteria for experts: ①A deeper understanding for the related field to the research; ②Bachelor degree or above, responsible for the management work of third grade class A hospital or high technical titles; ③ 10 years’or above work history in nursing management, nursing education, clinical care, clinical care and related fields;④Informed consent, willing to participate in the study of the subject.In the study,describing statistics was used to analyze demographic information of experts.Using Analytic Hierarchy Process Method According to the average importance assignment index of experts determining the scale of satty, constructing the judgment matrix.After checking the consistency of the post, we can establish the weight of each index.Results1.Through literature research and semi-structured interview, the team discussed and analyzed, the initial collection formed 55 Index system of the three aspects specialist nurse selected index,certification and evaluation, the first round of consultation questionnaire compiled after the expert detection.2.The selected experts are from four different provinces and different areas, So the result has better regional representative and extensive discipline representative. The first round of expert consultation questionnaire issued 23 copies,22 copies recovered, the second round of expert consultation questionnaire issued 22 copies,22 copies recovered.The effective recovery of two rounds of expert consultation questionnaire were: 95.6%,100%.In the first round,8 experts had proposed 21 amendments and recommendations. In the second round,1 experts had proposed 1 comments or suggestions that experts’enthusiasm was higher and tend to be consistent. when the experts’authority coefficient≥0.7, we believe that this result is acceptable reliability.The authority of the expert coefficient between 0.72 and 0.90. Conclusions as a result:The data has good authority and experts have high enthusiasm.So experts; judgment was based on practical experiences and theoretical basis.The Kendall’s W of the two round expert consultation were 0.310,0.332. After testing, proving it has statistical significance (P< 0.01).Conclusions as a result:The advice of experts has good coordination and concentration.3.Eventually,It formed 46 Index system of the three aspects specialist nurse selected index,certification and evaluation.Necessary of specialist nurses 8 items of selected indicators were:college graduate or above, major in nurse practice certificate, trained by the specialist areas system, intermediate or above technical titles, worked for 5 years or more,national journal published papers, English and computer.The certification department was the Ministry of health and Family Planning Commission;The certification method includes individual application, unified assessment, expert evaluation; The check interval was 2 years. Then, use of AHP to determine weights of indexes,clinical specialist practice was 0.381, professional attitude was 0.205, nursing management was 0.129, nursing research was 0.059,educational guidance was 0.096, the comprehensive quality and personal accomplishment was 0.129. In which clinical specialist practice had 10 items, professional attitude had 6 items,nursing management had 5 items, nursing research had 6 items,educational guidance had 4 items, the comprehensive quality and personal accomplishment had 5 items. The results showed that all of the index meet at logical consistency requirements.ConclusionsAn evaluation system that including 6 primary indicators and 35 secondary indicators,8 selected indicators and mode of qualification certification.The certification and evaluation system of specialty nurse provide a reference to the access certification and evaluation of specialty nurse.
