ObjectiveThis study aimed to construct the evaluation index system of county-level key clinical specialty and calculate the weights of index system in order to provide quantitative tools for the evaluation of the county-level key clinical specialty in Henan province.We also aimed to evaluate the county-level key clinical specialty in Henan province by using the index system and provide reference for selection and assessment of county-level key clinical specialty of Henan province.MethodsLiterature analytical method,focus group method and Delphi method were used to construct the index system.Averaging method and multiplication method were used to calculate index weights.TOPSIS method,RSR method and TOPSIS and RSR fuzzy set were used to perform comprehensive evaluation for the 22 county-level key clinical specialty of Henan province.Results1.Results of Delphi method.(1)A total of 20 questionnaires were issued and collected in 2 rounds of expert consultation.Expert positive coefficients in two rounds were 100%and expert authority coefficient was 0.888.(2)According to the importance,feasibility and sensitivity threshold of index system and expert suggestions in the first round of expert consultation,7 third-level indexes were deleted including the doctor number served as the editorial board of relevant medical journals and above,undertaking and hosting the number of meetings at the municipal level or above,the number of papers published in first-class and second-class journals,the number of national invention patent(including invention patent and practical appearance patent),the number of science and technology awards at the municipal level or above,the number of participating in writing books and average medical expenses of discharged patients in the latest three years.And 3 third-level indexes were added including the number of cooperation with other hospital at the adjacent to the area,whether there is normalized solutions to the new problem in the process of medicine and the implementation rate of the core system of medical quality and safety.We revised 2 third-level changing acquiring the number of new technology leading the municipal or above to acquiring or introducing the number of new technology leading the municipal or above and changing the number of papers published in journals to the total number of papers published in journals and participating in writing books.(3)The second round of expert consultation showed the weight of basic conditions,medical technique team,ability and level of medical service,medical quality status,research and teaching,economic management was 14.55%,19.25%,22.45%,20.70%,11.45%and 11.60%,respectively.2.Index system including 6 first-level indexes,16 second-level indexes and 57 third-level indexes was constructed.The 6 first-level indexes covered basic conditions,medical technique team,ability and level of medical service,medical quality status,research and teaching and economic management.3.Comprehensive evaluation of 22 county-level key clinical specialty in Henan province showed that the top 3 specialties were the R specialty,J specialty and P specialty.Conclusion1.We constructed an index system for County-level key clinical specialty in Henan province including six first-level indexes,sixteen second-level indexes and fifty-seven third-level indexes through literature analytical method,focus group method,Delphi method and averaging method.2.Based on the constructed the evaluation index system,we used TOPSIS method,RSR method and fuzzy set to perform a comprehensive evaluation of 22 county-level key clinical specialty in Henan province.The quantitative evaluation provided scientific basis for selection and evaluation of specialties in the future. |
