Objective: The purpose of this study is to construct a set of evaluation indexes for the competence of palliative and hospice care specialty nurses in line with China's medical system and cultural background,to provide scientific guidance for the professional competence development and occupational admission of palliative and hospice care specialty nurses in China.Methods: This study is a mixed study,which is mainly divided into four parts.1.Literature review: By consulting the relevant literatures of the palliative and hospice care nursing specialists at home and abroad,the elements of the competence of the palliative and hospice care nursing specialists were extracted,and the dictionary of competence of the palliative and hospice nursing specialists was formed.2.Qualitative research: Through purpose sampling and snowball sampling,select doctors,nurses,and nursing managers who are engaged in palliative and hospice-related work to conduct semi-structured in-depth interviews and collect data with participatory observation methods.Use continuous comparison method and content analysis method to address dataes.3.Delphi method: Two rounds of expert consultations are conducted by emailing 16 experts who meet the expert selection criteria,and the statistics and analysis of data are performed using Excel2010 and SPSS20.0 software.The degree of concentration and the degree of coordination of expert opinions test the scientificity of the two rounds of expert consultations;using the analytic hierarchy process to determine the weight of each item,and finally form an evaluation index of the job competence of the palliative and hospice care nurses.4.Reliability and validity test: According to the job competence evaluation indicators of the palliative and hospice care nursing nurses,a questionnaire for the palliative and hospice care nursing specialists was formed,and a convenient sampling method was used to select the research subjects who met the inclusion criteria to issue an electronic questionnaire.Statistical analysis was performed using spss20.0 software.The reliability was tested using internal consistency reliability,and the Cronbach'? coefficient was calculated.The validity was calculated using content validity and structural validity,and correlation analysis was performed to determine the reliability and validity of the index system.Results:1.Based on the literature review method and the competence iceberg model,the different characteristics of the specialty nurse's job competence were graded,classified and coded,and a competency dictionary was established,which finally formed 6 dimensions and 19 first indicators and 93 secondindicators;2.A total of 21 observation logs were completed,three groups of semi-structured face-to-face group interviews and nine groups of semi-structured face-to-face personal interviews have initially formed system including 6 primary indicators,31 secondary indicators,and 31 tertiary indicators.3.Delphe ‘s result is that the positive coefficient of experts is 100%,the overall authority coefficient of experts is 1,and the individual authority coefficient of experts ranges from 0.75 to 1.4,the concentration of experts opinions is relatively good,and the experts coordination rate is 0.451,P < 0.001,according to the opinions and suggestions of experts to modify the content of the index system,the formation of an evaluation index system for the job competence of nurses in palliative and hospice including 6 first-level indicators,29 second-level indicators,and 61 third-level indicators,and the weight of each index system is calculated according to the analysis method and the consistency test result is satisfactory.4.The results showed that the overall Cronbach's ? coefficient in the index system was 0.928,and the validity of the content of the entries was concentrated at 0.8 and 1.0,the content validity of the scale is 0.954,and the structural validity is better.Conclusion: A set of evaluation indexes for the competence of palliative and hospice care specialty nurses that meets the national conditions and cultural background of China has been formed,which includes 6 first-level indicators(knowledge,skills,personality traits,motivation,self-concept,social role),23 second-level indicators,and 61 third-level indicators.The index system of nurse job competence evaluation provides support for the job setting and certification,education,training and assessment standards of nurses in China.
