【Background & Objective】Some areas of our country have developed specialty nurse training in specific nursing fields such as intensive care unit,diabetes mellitus and stoma care etc. during recent years. It has been formed consensus that nurses work in specific nursing fields should go through specialty nursing training in our country now. The main training organizations contributing to these programs are nursing associations, the class 3-A hospitals and medical colleges. Some hospitals have trained and awarded certificate to their own specialty nurses by themselves. As clinic training base, the hospitals'specialization nursing practice training is the key point to guarantee the qualification of the specialty nurse training,but there is no access standards for these training bases in our country now, So the quality of the certified specialty nurse is uneven as the irregular levels and conditions of these training bases. Therefore, it is necessary to establish a scientific and systmatic access standards for specialty nurse clinical training base. Considering the physiological and psychological specificity in infants and children, and the specialization of the pediatric nursing field, our aim in this study is to get a generally receptible definition of pediatric specialty nurse which suit the conditions of our country, to set up a scientific, systematic and maneuverable evaluation indexes of pediatric specialty nurse clinical training base in the hope of providing reference for evaluation standards of pediatric and other special nursing fields to health care authorities who are in charge of nursing works, as well as building a foundation for pediatric specialty nurse training program.【Procedures&Methods】Based on literature review, comparative study and a practical investigation of some hospitals,and considering the realistic conditions of our country, we study out a definition of the pediatric specialty nurse, and some access evaluation indexes for pediatric specialty nurse clinical training base for selection. According to the content of our study, 35 experts who work in nursing supervision, clinical nursing and nursing education field were enrolled in the questionnaire investigation by the Delphi Method. Using the Likert-5 scale method to describe the expert's opinions of the importance for each item. The mean of important value beyond 3.5 and the variance coefficient less than 0.2 were used as a filtration standards of indexes.The final conclusions were drawn according to the above screen criteria and the team discussion, as well as the experts'comments. Then the analytic hierarchy process (AHP) was used to develop the relevant weights of the evaluation indexes. Mean of important value had been used to calculate the Saaty sign, and the multi-ranks judge matrix were established according to the AHP.Then the weight of each index was identified by the yaahp0.4.1.【Results&Conclusions】1. Reliability and consistency of Expert ConsultationThe qualitative analysis of the experts'basic situations (Specialty correlation, Age structure&work experience, Professional title&education level) and the quatitative analysis of the experts'authority(The whole auhtority coefficient of experts was 0.89±0.07)showed that the expert group had good authority and representation. The feedback rates of the two questionnaire processes were 82.9% and 94.1%, and there were 18(62.1%) and 7 experts(23.3%) proposed several important suggestions in the two questionnaire processes respectively. which imply that the experts response actively to our study. The minimum mean of evaluation indexes was 3.633, the maximal was 4.933, and the average was 4.403,all higher than 3.500, the 70% of full marks 5.000,. The Kendall's W coefficient of total indexes was 0.237(P<0.001),and CR<0.100. The examination on the indexes showed the convergence of experts'suggestion and reliability of this system.2.Construction of the Pediatric Nurse Definition & the Access Evaluation Indexes of Clinic Training BaseAll experts considered that training pediatric specialty nurse in China was needed (50%) or urgent needed (50%). 69.0% experts agree with the definition of pediatric specialty nurse and 9 experts give revising comments in the first questionnaire process ,and 78.1% experts agree with the definition in the second process, among 7 experts who disagree the definition of pediatric specialty nurse in our study, 4 experts actually consent this definition, but offering mild revision in literalness.After the two questionnaire processes, we establised the access evaluation indexes of pediatric specialty nurse clinical training base,which included 7 first grade items, 29 second grade items and 46 third grade items. Arranging in order by the weight , the first grade indexes were: the qualification of the clinical training teacher(0.289), training program schedule(0.215), the conditions of the training department(0.166), the construction of the department's nurses group(0.122), the training management organization(0.089), the hospital'general conditions(0.069), the training rules & regulations(0.051).3 Evaluation study on access indexes of pediatric specialty nurse clinical training baseUsing the constructed evaluation systems, PICU&NICU in a class 3-A pediatric specialty hospital authorized by Chongqing Health Bureau as the"Chongqing city pediatric ICU specialty nurse training base"and a pediatrics department in a class 3-A general hospital with high rank in pediatric field in Chongqing city were evaluated on the spot with every index of the indexes system, calculate the ratio of scores to full marks , and an analysis was conducted according to the evaluation results. The investigation study preliminary identified that the indexes system has good maneuverability.
