Flexible Use Of Smoked Plum And Smoked Plum Pill In "Guide To Clinical Medical Records(《临证指南医案》)"

ObjectiveSmoked plum is a herbal medicine and edible of traditional Chinese medicine, widely used by ancient physicians, but it’s effects are not from diarrhea, dysentery, Cough and so on. Ye Tianshi was a famous Doctor in Qing Dynasty, extremely respected for ZhangZhongJing. He specia lized in using and revising the classical prescription. Repeatedly emphasized the diagnosis and treatment method using by ZhangZhongJing in his medical case, which was called "Guide to Clinical Medical Records" (《临证指南医案》). Ye Tianshi grasped the essence of Zhang zhongjing and flexible change the classical prescription,but not rigidly adhered to the law in "Treatise on Febrile Diseases"(《伤寒论》)。Smoked plum first to be seen in the use of the" Treatise on Febrile Diseases ", which is one of the Herbs of Black plum pill. Ye Tianshi according to the law of Black plum pill, developing a series of Plum pill class party. This article make acomplete conclusion of using smoked plum by Ye Tianshi from "Guide to Clinical Medical Records"(《临证指南医案》).MethodIn this paper, through searching and recording the descriptions about the smoked plum with computer, and then analyzing the Way of thinking by Doc. Ye Tianshi, in order to widen the clinical thinking.The paper divided into two parts:(一)Study on smoked plum and smoked plum pill literature:This part including smoked plum and smoked plum pill’s medicine history, modern pharmacological research of smoked plum and smoked plum and the clinical application of smoked plum and smoked plum.(二) Study on application of smoked plum and smoked plum pill in "Guide to Clinical Medica] Records":To collect and search all the medical case treat by smoked plum included in the "Guide to Clinical Medical Records", recording the necessary data which is made of the species of smoked plum, diseases, syndromes, performances, pulses and tongue into the Excel, analyzing all the input data and obtain the result. Chose 12 clinic cases treat by smoked plum pill, including epigastric pain, cough, malaria, choking every nausea, heat stroke, emesis, vomiting of ascaris, dysentery, treated by smoked plum will obtain some satisfactory therapeutic results.Results1. Different types of smoked plum are used in the "Guide to Clinical Medical Records":there are eleventh types of smoked plum were used in the concerned medical cases. There are dark plum, pulp of Fructus Mume, fried smoked plum, Mume carbon. The dark plum and pulp of Fructus Mume are the most frequently used in the medical cases, two together is 74.33%.2. In the "Guide to Clinics Medical Records", there are twenty-seven of major diseases treat by smoked plum,which including malaria (23.52%), the wood over restrainstheearth (13.72%), diarrhoeaand vomiting of ascaris (10.78%), dysentery(7.84%).3. The wood restricting earth in the pathogenesis involved in applying smoked plum accounting for 45.16%, dampness-heat in middle jiao is 21.5%, cloudy wound and Jueyin syndrome are 12.9%. Four diseases together with a total of about 92.46%.4. The highest frequency of symptoms related to smoked plum in the "Guide to Clinical Medical Records" is nausea and vomiting, about 17.06%,and then followed by pain with 13.99%, chills and fever (11.60%), abdpmi-nal pain and diarrhrea is about 10.23%, poor appetite (9.21%), pectoral coerce suffering is full 8.19%.According to the Chinese Medical Theory of governance, those Syndromes which is found more frequent used with the smoked plum, like nausea and vomiting, abdominal pain and diarrhea, poor appetite are related to Spleen and stomach5. Red tongue and dry tongue is the major tongue picture related to smoked plum in the "Guide to clinical Medical Records"6. Taut pulse, soft pulse, thready pulse and rapid pulse are the major pulse related to smoked plum in the "Guide to clinical Medical Records". And the more frequent appeared pulses are taut pulse.8. Some other Chinese herbal medicine often to formulate with smoked plum in "Guide to clinical Medical Records" are ginseng, peonies, goldthread, tuckahoe, pepper, fresh ginger, Pinellia ternata, cassia twig, rhizoma zingiberis.9. The most frequently used dose of Smoked plum in the "Guide to Clinical Medical Records" are five"fen"(分). The highest dose of smoked plum is fifteen "qian" (钱). Three "fen" (分) is the lowest.10.
