Research For The Usage Of Aconite In "Guide To Clinica Medical Records(《临证指南医案》)"

Aconite has the function of temperature in cold-dispelling, save Huiyang inverse. Ye Tianshi was an expert in treating epidemic febrile diseases,whose masterpiece was "Guide to Clinical Medical Records"(《临证指南医案》)in Qing dynasty. The book uses a large number of prescriptions with aconite.There is extensive use of aconite in medical record and a lot of knowledge and experience about application of aconite.This valuable heritage should be attached importance to and In-depth study.This thesis concentrates in the usage of Aconite as included in the"Guide to Clinical Medical Records", altogether with searching and recording all the related descriptions about the aconite through computer, analyzing the thinking method of the author of the book-Mr. Ye Tianshi,in order that to set up the possible theatrical base for the modern clinical treatment as well as to have better curative effect for patients.The paper divided into three parts:(一) Study on Aconite literature:The part mainly summary and analysis literature of aconite from chemical constituents,poisonous and un-healthy effect,dose, efficacy, clinical application,modern pharmacological action,processing and compatibility of aconite.(二)Study on application of aconite in "Guide to Clinical Medical Records" To search all the medical case related to aconite which was included in the "Guide to Clinical Medical Records",collecting and inputting the necessary data including diseases,syndromes,performances, pulses and the formulation of different Chinese Medicines into the Excel,operating the professional statistical software-SPSS to analyze all the input data and obtain the result.(三)Clinical application of aconite practice by learn "Guide to Clinical Medical Records":The paper enumerates16complete clinic cases,including frequency of micturition,chest Bi-syndrome.bysubzonal,stranguria with turbid urine,hematochezia with hemorrhoids, phlegm and fluid,bitter taste in the mouth,consumptive disease, metrorrhagia,allergic rhinitis,tinnitus, arthralgia syndrome,insomnia, cold and apoplectic sequela were treated by aconite recipe,which have obtained some satisfactory therapeutic results.The results are as follows:1.The characteristics of using different types of aconite as recorded in the "Guide to Clinical Medical Records".In the "Guide to Clinical Medical Records",there is sixteen(16) types of aconite were appeared in the concerned medical cases. There are altered aconite root, light salted aconite root,prepared aconite root, concocting aconite root, light salted prepared aconite root,processing aconite root,frying black crude aconite roots,crude black concocting aconite root,small roasted aconite root,black concocting aconite root,crude light salted aconite root,crude concocting aconite root.The processing aconite root are the most popular used in the medical cases, both share is98.05%, including salted aconite root(45.45%),prepared aconite root(20.77%)and light salted aconite root(19.48%)2.Thirty-two of major diseases were treated using aconite in the "Guide to Clinics Medical Records".In the "Guide to Clinical Medical Records",according to the number of appearance of aconite in the diseases, as from more to less are aconite respectively,swelling(12.33%),dysentery(8.44%),emesis(7.14%),malaria(7.14%), phlegm and fluid (6.49%),etc.3.The deficiency of the middle energizer and turbid phlegm in the pathogenesis involved in applying aconite accounting for24.39%.The Yang deficiency of spleen and kidney, and yang deficiency and wind act is19.51%and13.17%respectively. These four diseases accounted for a total of about81.46%.4.In the major syndromes as related to aconite in the "Guide to Clinical Medical Records",the highest frequency of symptoms nausea and vomiting occurs, less than12.91%,followed by diarrhea(7.34%), aversion to cold or wind(10.63%), wind chill or heat(7.36%),poor appetite(9.11%), reverse Qi or symptoms are distention(10.12%), form and spirit decay(5.31%),pain (6.58%),and abnormal diaphoresis(5.06%).In the "Guide to Clinical Medical Records",according to the Chinese Medical Theory of Yi-Yang-Qi-Blood,those Syndromes like Qi deficiency, reverse Qi and Yang deficiency,involving both kidney and spleen are found more frequent used with the aconite.5.Major Pulse as related to aconite in the"Guide to clinical Medical Records" are respectively faint pulse, thready pulse, deep pulse.soft pulse,thin pulse,and feeble pulse.The more frequent appeared pulses are faint pulse,thin pulse, deep pulse,appears most times,11,9,8.6.In the "Guide to Clinical Medical Records",the aconite are found very often to formulate with some other Chinese Medicines which are poria cocos,panax ginseng, dried ginger, tract ylodes,ginger,white peony root, cassia twig, and magnolia officinalis,etc.7.Major tongue picture as related to aconite in the"Guide to clinical Medical Records"is tongue coating or white tongue.8.Major use dose of aconite in the"Guide to Clinical Medical Records"are seven "fen'(分)to one "qian"(钱)Three "qian"(钱)is the highest dose of aconite.9.The characteristics of aconite utilization:(1)hard and strong in nature of drug use with caution in sthenia;(2)warm and heat in nature,easy to harm the Yin-fluid;(3)Applied widely and flexible compatibilityThe article is first to systematically discuss diagnosis and treatment of drug laws of aconite in"Guide to Clinical Medical Records",summarize and analyze application of aconite's experience of Ye Tianshi.The research has a positive significance in guiding clinical application of aconite.
