ObjectiveIn order to compare the effectiveness of three different acupuncture treatment(acupuncture, acupoint injection and bloodletting) on the cervical spondylosis under real-world conditions, this study was designated base on comparative effectiveness research(CER). We use patient-reported outcomes (PRO) mode to compare the short-term and mid-term effectiveness of the three methods above.MethodsSubjects are recruited from acupuncture department in Guangdong Provincial Hospital of TCM.150 subjects were divided into 3 groups: standard control group(routine acupuncture group), acupoint injection group and bloodletting group. Case Report Forms (CRF) were filled in in four time points (before treatment, after treatment,1 month after treatment and 3 month after treatment). NPQ scale is main outcome indicators and SF-36 scale is supplementary outcome indicators. Statistical datas were analyzed by SPSS20.0 for windows.Results1.143 Subjects were recruited and divided into 3 groups:48 cases in standard control group(routine acupuncture group),48 cases in acupoint injection group and 47 cases in bloodletting group. The baseline was confirned by spss20.0 analysis for pre-treatment:sex(P>0.05),age(P> 0.05), duration of course (P>0.05), NPQ(P>0.05), SF-36 (P>0.05) showing no significant differences.2. Statistics show that there are significant differences(P<0.05) in NPQ Within-Subjects Contrasts, which mean that both 3 groups have definite effect in relieving neck pain. Then NPQ Between-Subjects Contrasts show that there are not significant differences (P> 0.05) between standard control group and acupoint injection group, but there are significant differences (P< 0.05) between bloodletting group and the 2 groups above. These results suggest that effects of standard control group and acupoint injection group are similar but bloodletting group is inferior to the 2 groups above.3. Statistics show that there are significant differences (P<0.05) in SF-36 Within-Subjects Contrasts, which mean that both 3 groups have definite effect in improvement of quality of life. Role-Physical (RP) and Vitality(VT) Between-Subjects Contrasts show that there are significant differences(P<0.05),which mean that acupoint injection group has better effect in those 2 aspects than the other 2 groups.ConclusionAcupoint injection group and standard control group have similar effect in relieving neck pain. Acupoint injection group has better effect in improvement of Role-Physical(RP) and Vitality (VT) than the other 2 groups. Bloodletting group and standard control group have similar effect in improvement of quality of life. But bloodletting group has poor effect in relieving neck pain compared with the other 2 groups.
