ObjectiveThrough the study of the pathogenesis and treatment of cervical spondylosis of nerve root type in traditional Chinese and Western medicine,the understanding of cervical spondylosis of nerve root type is deepened.Through the clinical randomized controlled trial,the patients with cervical spondylosis of nerve root type were treated with basic acupuncture combined with bloodletting and basic acupuncture respectively.The difference of curative effect between the two groups was compared,and the mechanism of action was analyzed,leading to an exploration of a new method of treatment.HethodsAll the cases collected in this study were volunteers recruited by Foshan Jianxiang Hospital.The specific cases were collected from 2017 to 2018.A total of 66 patients with cervical spondylosis of nerve root type were collected.The 66 patients were randomly divided into control group(basic acupuncture group)and observation group(basic acupuncture combined with bloodletting group),except for the shedding part,the maximum age was 64 years old and the lowest age was 31 years old.There were 13 males and 47 females.The longest course of disease was 2 years and the shortest was 1 month.The acupuncture acupoints of the observation group(basic acupuncture combined with bloodletting group)and the control group(basic acupuncture group)were all referred to the third edition of Acupuncture Therapy published by China Traditional Chinese Medicine Publishing House,edited by Gao Shuzhong.The acupoints used were:neck Jiaji(double),Tianzhu(double),Ashi Point,Quchi(double),Waiguan(double),Houxi(double),Shenmai(double).In the acupuncture combined with acupuncture and bloodletting group(observation group),the acupoints selected with reference to acupuncture and moxibustion therapy:were Dazhui,neck Jiaji(double),Dazhu(double),Jianjing(double),acupuncture points with trigonous acupuncture,and then cupping and bloodletting,one acupoint at a time.The observation group was treated with basic acupuncture combined with acupuncture and bloodletting,while the control group was only treated with acupuncture,and the acupoints were the same as those in the observation group.Treatment course:every Tuesday,Thursday,Saturday afternoon treatment,a total of three times a week,4 weeks as a course of treatment,a total of 1 course of treatment before and after treatment.Outcome measures included:cervical spine function,anxiety symptoms,pain degree and quality of life were evaluated by Tanaka Jingjiu Cervical symptom scale(20 points),GAD-7 extensive anxiety scale,McGill pain questionnaire(including pain rating index(PRI),visual pain score(VSA),and current pain status(PPI)and SF-36 production quality scale.The curative effect of the two groups was evaluated before and after treatment,and the clinical efficacy of the two treatments was compared.Mean±standard deviation for measurement data means.In accordance with the normal data,the matched sample t test was used to compare the scores in the group,and two independent sample t tests were used to compare the scores between the groups;otherwise,the matched symbolic rank sum test was used to compare the scores in the group,and the rank sum test of two independent samples was used to compare the scores between the two groups.T test or rank sum test was used to compare the score difference before and after treatment.The counting data were expressed by frequency n(constituent ratio%)and compared statistically by Rank test.P?0.05 was statistically significant.Results1.Sample equilibriumIn this study,66 qualified patients were randomly divided into two groups.There were 33 cases in the observation group,3 cases in the observation group,33 cases in the control group and 3 cases in the control group.After statistical analysis,there was no significant difference in the distribution of sex and age between the observation group and the control group(P>0.05),suggesting that the sex and age of the observation group and the control group were balanced and comparable.2.The comparison of symptoms and function evaluation between the control group and the observation group before and after treatment.There was no significant difference in the score of cervical spondylosis scale between the two groups before and after treatment(P<0.05).After treatment,the scores of cervical spondylosis scale in the two groups were higher than those before treatment(P<0.05).The difference between the two groups before and after treatment showed that the difference before and after treatment was positive,indicating that the score of cervical spondylosis scale after treatment was higher than that before and after treatment-The increase of the observation group(8.43±2.86)was higher than that of the control group(6.87±1.66).There was significant difference between the two groups,which was statistically significant.Therefore,it can be considered that the acupuncture in the observation group is higher than that in the control group(6.87±1.66).Combined with acupuncture and bloodletting,it was better than the control group in improving the symptom score of cervical spondylosis,and the effect of relieving cervical spondylosis symptoms was more obvious.3.Comparison of comprehensive pain assessment between control group and observation group before and after treatment.(1)Comparison of McGill-PRI scoresThere was no significant difference in McGill-PRI score between the two groups before treatment(P<0.05).After treatment,the McGill-PRI score of the two groups decreased,and there was significant difference between the two groups before and after treatment.Through the difference between the two groups before and after treatment,the difference between the two groups before and after treatment showed that the difference before and after treatment was negative,indicating that the McGill-PRI score decreased after treatment.The decrease of the observation group(10.47±4.785)was higher than that of the control group(7.63±4.319).There was significant difference between the two groups(x 2?4.705,P<0.05)It can be concluded that acupuncture combined with bloodletting in the observation group is better than basic acupuncture in the control group.(2).Comparison of VAS scoresThere was no significant difference in VAS score between the two groups before treatment,and there was no significant difference between the two groups before and after treatment.After treatment,the VAS score of the two groups decreased,and there was significant difference between the two groups before and after treatment.Through the difference between the two groups before and after treatment,the result showed that the difference before and after treatment was negative,indicating that the VAS score decreased after treatment,and the decrease range of the observation group(4.83±0.913)was higher than that of the control group(3.53±0.776).There was significant difference between the two groups(x 2?4.85,P<0.05),so it could be considered that the VAS score was significantly higher than that in the control group(P<0.05).On the other hand,acupuncture combined with acupuncture and bloodletting in the observation group was superior to the basic acupuncture in the control group in reducing VAS score,and the symptoms of intuitionistic pain were more obvious.(3).Comparison of PPI scoresThere was no significant difference in PPI score between the two groups before treatment,and there was no significant difference between the two groups(P>0.05).After treatment,the PPI score of the two groups decreased,and there was significant difference compared with that before treatment(P<0.05).The difference between the two groups before and after treatment showed that the difference before and after treatment was negative,which indicated that the PPI score decreased after treatment.At the same time,the decrease range of PP in the observation group(1.53±0.571)was higher than that in the control group(0.97±0.183).There was a significant difference between the two groups(x 2?4.862,P<0.05).Therefore,it can be considered that the decrease of PP in the observation group is significantly lower than that in the control group(P<0.05).In terms of I score,acupuncture combined with acupuncture and bloodletting in the observation group was better than the basic acupuncture in the control group,and the symptoms of cervical spondylosis were alleviated more obviously.(4).Comparison of GAD-7 extensive anxiety scoreThere was no significant difference in GAD-7 score between the two groups before treatment,and there was no significant difference between the two groups.After treatment,the GAD-7 score of the two groups decreased,and there was significant difference between the two groups before and after treatment.The GAD-7 score of the two groups decreased before and after treatment.At the same time,the decrease range of the observation group(5.50±2.389)was higher than that of the control group(3.50±1.592).There was a significant difference between the two groups(x 2?4.801,P<1.592).Therefore,it can be considered that the GAD-7 score of the two groups is statistically significant.GAD-of acupuncture combined with acupuncture and bloodletting in observation group's score was better than the basic acupuncture in the control group,and the effect of relieving anxiety disorder was better.(5).Description of quality of lifeThere was no significant difference in quality of life score between the two groups before treatment(P>0.05),but there was no statistical significance.After treatment,the quality of life score of the two groups increased,and there was significant difference compared with that before treatment(P<0.05).Through the difference between the two groups before andafter treatment,the difference before and after treatment showed that the difference before and after treatment was positive,indicating that the score of quality of life after treatment increased.At the same time,the increase of the observation group(250.30±100.203)was higher than that of the control group(175.06+130.52),and there was significant difference between the two groups(x 2?7.221,P<0.05).Therefore,in the aspect of quality of life score,the quality of life score of acupuncture combined with acupuncture and bloodletting in the observation group is better than that in the control group,and acupuncture combined with acupuncture and bloodletting can significantly improve the quality of life.(6).Comparison of clinical efficacy The cure rate was 16.7%,the effective rate was 43.3%,the effective rate was 40%,the ineffective rate was 0%,the total effective rate was 100%,the cure rate was 3.3%,the effective rate was 36.7%,the effective rate was 46.7%,the ineffective rate was 13%,and the total effective rate was 87%.Therefore,it can be considered that the curative effect of acupuncture combined with acupuncture and bloodletting is better than that of conventional acupuncture.ConclusionCompared with conventional acupuncture,acupuncture combined with collateral bloodletting is more effective in the treatment of cervical spondylosis of nerve root type,which can be reflected in improving the degree of pain,restoring the physiological function of cervical vertebrae,improving the quality of life and improving anxiety symptoms.
