Injection Of "chuan Xe Zhi" In Patients With Chronic Obstructive Pulmonary Disease With Acute Aggravating Period Of Clinical Curative Effect Observation

The paper is composed by review,clinical research and animal experiment.The part of reviewSystenmatic summarize the clinical reaserch of the reason, pathologcal and phasiology mechanism,and the newest treatment of AECOPD of recent5years.Systenmatic summarize the etiology,pathogenesis, the features and developing principles of TCM symptomatology,syndrome differentiation and treatment of traditional Chinese medicine of recent10years.The part of clinical researchObjective To evaluate the clinical efficacy of using Chuankezhi injection in the treatment of different type patients of Acute exacerbation of chronic obstructive pulmonary disease(AECOPD),to evaluate value of benefiting qi and warming Yang tharapy in the treatment of AECOPD. Methods AECOPD patients that fit the diagnosis standard of the case, were randomly divided into treatment group and control group.96cases was take into the reasch, but8cases was losed, finaly, there were88cases that have statistical significance.The treated group have45cases and the control group have43cases.The control group is given therapies based on the standardize treatment of "Guide of COPD(2007)’’ including anti-infection, relieving wheezing, mechanical ventilation if necessary, and Chinese compound recipe prescribed based on the different type of Triditonal medicine. Patients in the treated group were treated with the tharapy of the control group, and plus Chuankezhi injection,4ml,by musle injections, twice a day,for a week’s course.The clinical efficacy is evaluated by the symptom such as Coughing, Expectoration, Wheezing and chest tightness. And index of laboratory examination including blood routine,CRP and arterial blood gas analysis. Respectively observe thiese indicaters at the1st day, the third day and the7th Day.Results The symptoms and index of laboratory examination of both groups can be alleviated.But in the aspacts of the descent level of Wheezing, chest tightness and the improvement level of PO2,the treated group is better than the control group(P<0.05);the comparison of the4subgroups of different TCM syndrome type patients show that the treated group is better than control group in the descent level of total score of clinical smptom; the descent level of total smptom score of different TCM syndrome type patients in the treated group have no difference(P>0.05).The comparison of single sympotom of the4subgroups of different TCM syndrome type patients show that the treatment group is better than the control group at some aspects such as Wheezing.Conclusion Chuankezhi injection can improve the clinical efficacy of AECOPD, PO2of patients in the treated group can be better improved.Benefiting Qi and warming Yang method is helpful to AECOPD patients of each TCM syndrome type.The part of animal experimentIn this part,the author introduced how to establish an experimental COPD rat model and the matters need attention in this course.
