Objective:To study the correlation between job satisfaction of ICU nurses and moral dilemma and the coping styles, then provide proof on how to improve ICU nurses’ job satisfaction and stability.Methods:General information questionnaire, moral dilemma questionnaire, coping style questionnaire and job satisfaction questionnaire were used to investigate107ICU nurses in four Affiliated Hospitals of Shandong University between May and Oct in2012. The SPSS15.0software package was used for statistical analysis.Results:1The overall job satisfaction mean score of107ICU nurses was3.37±0.29, slightly higher than the medium level. The relationships with colleagues dimension average score was4.03±0.38; The management dimension average score was3.51±0.45; the work dimension average score was3.39±0.44; the workload dimension average score was3.34±0.48; the personal growth and development dimension average score was3.34±0.77; the work authorized dimension average score was3.19±0.66; the family/work balance dimension average score was2.75±0.89; the wages and benefits dimension average score was2.63±0.73.2Comparison between different characteristics of ICU nurses and job satisfaction: The average scores in overall job satisfaction and management, wages and benefits, and family/work balance dimensions were higher in ICU nurses younger than30years old than those older than30years old (P<0.01; P<0.05); The average scores in overall job satisfaction and workload, work authorized and family/work balance dimensions were higher in ICU nurses work less than1year than those work more than one year (P<0.01); The average scores in overall job satisfaction and wages and benefits and personal growth and development dimensions were higher in ICU nurses with lower educational background (P<0.05); The average scores in overall job satisfaction and management, work,wages and benefits, personal growth and development and work authorized dimensions were higher in ICU nurses with higher monthly income (P<0.01; P<0.05); The average scores in work and personal growth and development dimensions were higher in unmarried ICU nurse (P<0.05).3The average scores of ICU nurses’moral dilemma and personal responsibility dimension were negatively correlated with the overall job satisfaction score and all the other eight dimensions. The unmet patient needs dimension score was negatively correlated with the work authorized dimension. The conceal dimension average score was negatively correlated with the overall job satisfaction score, management,wages and benefits and personal growth and development dimensions.4ICU nurses coping style score and its correlation with job satisfaction:The average scores of coping styles as problem-solving, rationalization, help, fantasy, retreat, and remorse were0.6768±0.1882,0.4469±0.1598,0.6383±0.2926,0.2617±0.1315,0.2721±0.1334,0.2468±0.1501. Problem-solving coping style score was positively correlated with management, workload, relationships with colleagues, wages and benefits, personal growth and development, work authorized, family/work balance dimensions (P<0.01); help coping style score was positively correlated with wages and benefits dimension (P<0.05); fantasy coping style score was negatively correlated with the overall job satisfaction, work itself, personal growth and development and the family/work balance scores (P<0.01; P<0.05); retreat coping style score was negatively correlated with the overall job satisfaction, wages and benefits and personal growth and development scores (all P<0.01); remorse coping style score was negatively correlated with the overall job satisfaction, management, workload, wages and benefits, personal growth and development, work authorizes, family/work balance scores (P<0.01; P<0.05).5Our multiple regression analysis showed that overall job satisfaction was related to the moral dilemma score, the nurses’ personal responsibility, problem-solving coping style, retreat coping style, monthly income, level of education, conceal factor and self-blame coping style.(R2adj=0.833) The management dimension influencing factors were the nurses’personal responsibility and conceal factor (R adj=0.291); the wages and benefits dimension influencing factors were monthly income and the conceal factor (R2adj=0.395); the personal growth and development dimension influencing factor was monthly income (R adj=0.368); the work authorized dimension influencing factors was work time(R2adj=0.225); the family/work balance dimension influencing factor was work time (R adj=0.258).Conclusions:1ICU nurses’job satisfaction was at middle level. The scores of wages and benefits dimension and the family/work balance dimension were lower than the other dimensions. So improving nurses’wages and reasonable arrangements for the contradiction between the working hours and family affairs were two ways to enhance ICU nurse job satisfaction.2The ICU nurses are facing high moral dilemma pressure, especially the personal responsibility dilemma, and the higher the degree of moral dilemma the lower the job satisfaction. Our study indicates that the managers should pay attention to the pressure the clinical moral dilemmas bring to nurses. And we should develop support and communication systems, improve the work environment for nurses so as to reduce moral dilemma pressure and improve job satisfaction.3ICU nurses usually take problem-solving and help coping styles. And our study indicates that mature coping style can improve job satisfaction of ICU nurses, immature coping style can reduce job satisfaction of ICU nurses. Guidance for nurses on how to take active and mature coping styles to deal with the problems are important aspects to improve ICU nurse job satisfaction.4Moral dilemma and problem-solving coping style are important influencing fators of ICU nurses’job satisfaction. Therefore alleviate the pressure brought by the nurses’ moral dilemma from different perspectives, and guide nurses on how to deal with the problems faced by mature response, are important aspects to improve ICU nurses’job satisfaction, and so on to the entire nursing profession.
