Background With the rapid development of medical technology and people’s growing demand of health, in the field of clinical nurse appeared a group of experienced, knowledgeable nurse specialists to provide high quality care service, they can solve the difficult problems in care that normal nurses can’t solve and the doctors have no time to, reducing patients’suffering. Overseas studies have shown that the services of clinical nurse specialist can reduce clinical complications and shorten hospital stay of patients. At present, there is a native theoretic discussion on specialty nurses’ training, certification and utilization to a certain extent refer to the experience of development of clinical nurse specialist in other country. Meanwhile, parts of the hospitals are trying to start practical exploration by following models abroad. However, because of the lack of relative reference of theory to bring the practice of specialty nurses into line.so far, the specific job responsibilities and access standards specialty nurses still need to be discussed.Aim To explore the role competency and practice patterns of the existing specialty nurses in Shanghai. To form the standards of the role, competency and practice model of specialty nurses in theory.Methods This study is an qualitative research. Firstly, interview 8 nursing administrators of Shanghai top three hospitals through snowball sampling with semi-structured interview employed. Secondly, select 9 specialty nurses and interviewing again by semi-structured method combining the result of research. After that, start non-participant observation between four of them. And then, sum up current role, competency and practice patterns of specialty nurses. At last, on the basis of the steps above, organize written advices of experts, finally, a conclusion in theory of current role, competency standards and practice model of specialty nurses was made.Results The prototype practice of specialty nurse exists in Shanghai. And various of hospitals start their practical exploration of clinical care following the foreign patterns, but their practical forms, job obligations and methods are different from each other. However, they have in common in terms of the role and competence performance, the specialty nurses provide patients with high quality care in the clinic, to assist team members in solving problems, and take part in patient management. On top of that, they gain the recognition and trust from patients, promote the advancement of nurses and value themselves, and get the support from the nursing supervisor, thus, contribute to the development of nursing. The researchers organized the roles, competency standards and practice model of specialty nurse theoretically in our own current national conditions.Conclusion The development of specialty nurse in China has its necessity and feasibility, but it must on the basis of integrating our existing resources of specialty nurses and forming a practical standards of specialty nurses, and then guide the utilization and training of specialty nurses. The role, competency standards and practice model concluded in theory by this research has a certain instructional significance, but still requires further refinement and verification with the actual characteristics of different specialty.
