Survey On The Seiures And Drug Teatment In Patients With Epilepsy In China

Objectives:To know basic situation of epilepsy treatment of China; to clear epilepsy seizure types, causes and its impact; to know overall situation of drug therapy in patients with epilepsy of China, and the differences between various types patients.Methods:Epilepsy patients who went to epilepsy specialist clinics or hospitals and treated with antiepileptic drugs during January 2009 to March 2010 in 26 provinces of Chinese mainland except Qinghai, Tibet and Xinjiang were recruited, a total of 7619 cases. Survey was adopted to investigate the objects. Basic information, seizures condition and drug treatment information were collected. Excel 2003 and SPSS13.0 statistical package were used for statistical description and statistical analysis.Results:1. In Jiangsu, Zhejiang and Guangdong Province, most patients seek treatment for epilepsy. In all treared patients, 4323 cases were male patients (56.7%), while 3296 cases were women (43.3%). 0~10 years old patients and 11~ 20 years old patients account the largest proportion, 30.37% and 21.95%, respectively. With increasing age, the smaller proportion seeked treatment.2. 3148 cases (41.32%) were general seizures while 3710 cases (48.69%) were partial seizures. Newly diagnosed epilepsy patients accouted for a larger proportion of 61.56%.3. The vast majority of epilepsy patients have no clear cause, which percentage was 63.77%; only 24.23% of epilepsy patients have a clear cause. Between the 1846 cases who have clear cause, brain trauma accouted the largest proportion(32%) in the six kinds of case; the next were encephalitis and birth trauma, which the percentage were 18% and 17%; the proportion of genetic and brain tumors were the smallest,only 11 % and 6%; other causes accounted for 11%.4. 48.6% epilepsy patients thought that seizures almost have no impact on their school life; 38.9% thought that seizures will affect their work, study or growth and development; 12.5% thought that seizures make them can not take care of themselves.5. Currently 74.77% epilepsy patients are using one AEDs; 23.32% patients are using combination therapy. 52.82% patients of single-drug treatment are using conventional AEDs. 55.54% patients of combination therapy are using conventional AEDs and new AEDs.6. 43.8% patients'desired therapeutic target was not seizure anymore; 26.3% was no adverse reactions; 23.1% was to reduce seizure and adverse reactions to a minimum level, while 6.8% was to improve quality of life.7. Only 52.79% epilepsy patients are satisfied with the current drug treatment, 19.67% patients are not satisfied with the current drug treatment. During the patients who do not satisfied 15.1% is because the number of seizures has not decreased; 67.3% because the number of attacks is not full controlled; 17.5% because can not stand drug side effects.8. The choice of drug treatment and therapeutic drugs in different seizure types was significant difference (P<0.05). The treatment way a 78.1% partial seizures patients were single-drug treatment, significantly higher than general seizures patients(74.6%); patients with partial epilepsy(38.5%) using new antiepileptic drugs were significantly more than general seizures patients(32.4%).9. The treatment goals was significant difference between general seizure patients and partial seizures patients (P<0.05). Compared with generalized seizures, more partial seizures hope do not seizure anymore (44.4%) and life quality can change (7.6%). Patients with satisfaction and dissatisfaction between two types patients was no significant difference (P>0.05), about 72% to 73% epilepsy patients are satisfied with the current status of drug treatment. The vast majority patients not satisfied with treatment is because the number of attacks is not full controlled. Conclusions:1. The more economically developed provinces, the more epilepsy patients went to seek treatment. More male patients went to seek treatment than female patients. With age increased, the smaller proportion seeked treatment. Newly diagnosed epilepsy patients accouted for a larger proportion than referral diagnosed epilepsy patients.2. 24.23% epilepsy patients have clear cause. Between the 1846 cases who have clear cause, brain trauma accouted the largest proportion; the next were encephalitis and birth trauma.3. More epilepsy patients are using one AEDs, and more patients of single-drug treatment are using conventional AEDs. During patients who are using combination therapy, more patients are using conventional AEDs and new AEDs.4. 43.8% patients'desired therapeutic target was not seizure anymore. 19.67% patients are not satisfied with the current drug treatment, and most is because the number of attacks is not full controlled.5. The percentage of partial seizures is higher than general seizures. The percentage of partial seizures patients who choose single-drug treatment is higher than general seizures patients. Patients with partial epilepsy using new antiepileptic drugs were significantly more than general seizures patients. Compared with generalized seizures, more partial seizures hope do not seizure anymore and life quality can change. Patients with satisfaction and dissatisfaction between two types patients was no significant difference.
