Investigation On The Present Situation Of Inhalation Administration In Some Community Health Service Centers In Shanghai

ObjectiveInhalation drug delivery is a way of fast delivery,because of rapid onset,easy to use,less adverse reactions,high bio-availability,and now getting more and more popular,more and more widely used.The grassroots community in our country still has not received enough attention and popularization,the clinical application also has many problems to be raised,such as patient awareness is not in place,the operation is not standardized,doctors lack of professional training,inadequate education and so on.This paper makes a questionnaire survey on the status quo of inhaled drug delivery in 3 communities in Shanghai,and on the basis of finding relevant literatures,discusses the superiority,application and latest research progress of inhaled drug delivery system,aiming to provide guiding help for more standardized application and further popularization of inhaled drug.Methods1.According to the current clinical application of inhalation administration,a self-designed questionnaire was used to conducted a survey sample of medical staff and outpatients in three community health service centers,which was used to understand the degree of awareness and rational application of inhaled drugs in medical staff and patients.2.On the basis of the survey,by searching PubMed,Cochrane library,EMbase,Chinese biological medical discdatabase,CNKI,VIP and Wanfang database to collect relevant documents and monographs published at home and abroad.3.After reading and finishing related literatures,through the tutor's meticulous guidance,the questionnaire survey results are summarized,summed up and analyzed,and the advantages of inhalation,the selection of devices,methods of use and research progress are discussed.Results1.Analysis of present situation of inhalation administration106 community medical staff all in the survey,do not understand and do not understand completely inhalation has a total of 50,accounting for 47.2%;believe inhaled medication safety has a total of 80,accounting for 75.5%;believe inhaled medication has reliable curative effect has a total of 85,accounting for 80.2%;know 3 or more drugs by inhalation there are 11 people,accounted for only 10.4%;know the 3 suction device has 10 people,accounted for only 9.4%;The medical personnel received 1 knowledge training for 77,accounted for 72.6%,has not received the related training of 29,accounted for 27.4%;2 of the patients who had been guided and missionary on several occasions,accounted for only 1.9%;only one or no of patients with inhalation of drug related guidance and education of the total of 53,accounting for 50%;multiple active follow-up and evaluation of patients with 2 health care workers,accounting for 1.9%,and no follow-up or assessment for 25,accounting for 23.6%.Of all 417 patients surveyed,the knowledge and cognition of inhaled drugs were very deficient,not knowing and totally not aware of 379,accounted for 90.9%;believed that inhalation medication safe and reliable 310,accounted for 74.3%;it is believed that the efficacy of inhaled medication is reliable in 247,accounted for 59.2%.Aware of the very few inhaled drugs and devices,know more than 3 kinds of inhaled drugs only 14,accounted for only 3.4%,know the 3 suction device 1,accounted for only 0.3%;received 1 or more related to inhalation of the guidance and education of 61,accounting for 14.6%,no received medication The guide and education of 356,accounting for 85.4%.In 108 patients who have been used inhalation drugs by oneself or around the person,has been instructed only 4,accounted for 3.7%,their own reading manuals used 66,accounted for 61.1%,according to their own ideas randomly used 38,accounted for 35.2%;in the process of using a local adverse reaction of 81,up to 75%,no adverse reaction of 27,accounted for 25%.The survey results show that three community health service centers of medical staff and patients with the knowledge and cognition of the inhalation of the general lack of drug delivery,the theoretical knowledge of the inhalation of drugs and the operation of the device is very low awareness of the rational and standardized education,inhalation medication lack of local side effects.To some extent,it reflects the need to improve and enhance the application of the inhalation drug in the grassroots community.According to the analysis of community survey,and summarized the deficiency,and on this basis,by searching the relevant literature,describes the inhalation advantages,features and how to use reasonably,to improve inhalation in the understanding of the importance and the implementation of the specification.2.The advantages of inhalation drug deliveryInhalation combined with the characteristics of aerosol technology and respiratory system,has a rapid onset,less dosage,high bio-availability,easy to use,less toxic side effects,patients with good compliance and many other unique advantages.3.The types of inhalation devicesInhalation device can be divided into: metered dose inhaler and spacer,dry powder inhaler and nebulizer.Dry powder inhaler is divided into passive and active,and passive dry powder inhaler device can be divided into single dose and multiple dose inhaler inhalation device.The nebulizer includes jet atomizer,ultrasonic atomizer and vibration sieve atomizer.4.The selection and use method of devicesMedical staff should consider the patient's own characteristics combined with the actual situation,so as to select the most appropriate drug delivery device.Medical personnel should be familiar with inhaled medications and master the operation of the device and the medicine suction method for patients with various characteristics of inhalation,individualized treatment,according to the guidance of absorbing correctly,to achieve the most ideal therapeutic target.ConclusionInhalation has the advantages of high efficiency,low toxicity,convenient,quick.To improve and perfect the grass-roots community medical staff and patients inhaled medication knowledge and drug delivery device for rational use of education and guidance,has very important practical significance to strengthen the clinical curative effect of inhaled drugs and expand the application prospect.With the continuous development and application of new inhalation devices and formulations,it will lead to a breakthrough in the treatment of many diseases.
