Objective:To investigate the clinical effect and adverse reactions of Wu She Xiao Bi decoction combine with scale-inoculation in the treatment of psoriasis vulgaris。Methods:80 patients were enrolled from department of dermatology of HuBei hospital of Chinese Medicine.All patients fit to the criteria of psoriasis vulgaris. These patients were assigned randomly into two groups in a 1:1 ratio. All patients received Wu She Xiao Bi Decoction orally for 60 days.Meanwhile, the cases from the treatment group and the control group received the scale-inoculation treatment and the catgut-inoculation treatment respectively. The duration of the treatment was two months. The treatment group received the treatment once every 20 days, three times in total; the control group received treatment once every 10 days, six times in total.When the patients returned to the hospital to receive Chinese medicine, their PASI scores were then calculated and recorded. They were also asked to assess the degree of itching and other adverse conditions they may have met. After treatment, the clinical efficacy were divided according to the change of PASI scores into cure,excellence, improvement and uselessness. Those patients who have recovered were tracked once every a month for twelve months. In the period, we recorded the rate of recurrence in both of treatment group and control group, the occurring time and the PASI scores of lesions. The PASI scores before and after treatment were statistically analyzed for both treatment group and control group. The degree of itching,before and after treatment,was also compared. Meanwhile, the efficacy of both two groups were analyzed and the rate of recurrence were also compared between two groups. SPSS11.0 software was used for statistical analysis. The adverse reaction in two groups were recorded, including main symptoms, the occurring time, the therapy used to treat these adverse reaction and the results.Results:1. There were no significant differences in age,gender,duration,PASI scores and the degree of itching between the treatment group and the control group (P>0.05)2. The PASI scores and the degree of itching of two groups have improved significantly after treatments (P<0.01).However, the effective rate has no significant differences between the two groups (P>0.05), it was obvious both two therapies have good clinical efficacy.3. Three patients relapsed in the treatment group, the average time was (9.33±1.15) months after treatment, meanwhile, eight patients relapsed in the control group, the average time was (6.63±1.85) months after treatment. The rate of recurrence was significant differences between both groups at six and twelve months respectively (P<0.05), the treatment groups was better than the control group in the aspect of delaying the recurrence of the disease. The same treatment as before was still helpful for the recurring patients.4. It was observed that both groups had one patient suffering from gastrointestinal discomfort respectively during the course of the treatment, including nausea, vomiting and diarrhea. These symptoms disappeared spontaneously after the treatment was stopped. In addition, it was noted that a nodule appear at the area of inoculation on the upper arm of one patient from treatment group, but the patient feel no discomfort. In the control group, another patient appear the same condition on the back and leg. These nodules were not treated, and disappeared after 8 weeks spontaneously, there was no Koebner phenomenon or other adverse reactions between the treatment group and the control group. The results from laboratory were also normal.Conclusion:It was effective the scale-inoculation combine with Wu She Xiao Bi decoction in the treatment of psoriasis vulgaris.Main symptoms and signs of psoriasis were alleviated, such as the size of the lesion, the thickness of the scales and the degree of itching. To a certain extent, the therapy can delay the recurrence of the disorder. There was no occurence of Koebner phenomenon or other significant adverse reactions after treatment. The clinical efficacy and safety are confirmed, the therapy also provides new clues and ideas for the treatment of Psoriasis Vulgaris using the immunization route.
