| Background and Objective:Psoriasis is one of common recurrent erythroderma desquamativums,usually called cowleather-like ringworm.It is named "Baibi" in traditional Chinese medicine. Among all patients with psoriasis,psoriasis vulgaris accounts for 97.98%.However, there is still no effective therapy.Xian ling xiao yin capsule along with routine treatment was applied for treating of 179 cases of psoriasis during June 2007 to September 2008 and acquired satisfactory healing effects.This medicine has the function of clearing away heat and toxic materials,and dispersing pathogenic wind and moistening dryness,and the characteristics of good efficacy,low recurrence and negative effects.This study aims to explore the mechanism of treatment of psoriasis by Xian ling xiao yin capsule and to provide clinical application of it with experimental basis.After 4 weeks of treatment,nepheloemetery was used to detect the index of CRP; radioimmunoassay(RIA) was applied to find out the clinical efficacy of psoriasis vulgaris.Methods:The 179 patients were randomly divided into two groups:treatment group(92 cases,aged 30.4±2.6,including 58 ones with blood-heat type and 34 ones with blood deficiency) and control group(87 cases,aged 32.3±2.9,including 52 ones with blood-heat type and 35 cases with blood deficiency).The treatment group was treated with Xian ling xiao yin capsule along with routine treatment,and the control group was given routine treatment.During 4 weeks of treatment of differentiation of symptoms and signs,PASI scores were used to evaluate curative effects and biochemical inteqral was made.CRP was measured among all patients,and IL-6 was measured by radioimmunoassay.Results:Some pathological changes such as hyperkeratosis,paraceratosis,acanthosis, anqiotelectasis and obvious single karyocyte infiltration can be observed.After treatment,histopathology of skin lesions is greatly improved and significant differences were found in biochemistry intequras(P＜0.05).It was found that PASI of treatment group was obviously lower than that in control group.There was obvious difference in IL-6 between the treatment group and control group and inter block contrast,with 215.38±85.23,208.78±107.65 before experiment and 101.32±57.61,135.97±78.26 after experiment in treatment group and experiment group respectively.IL-6 was 9.3±4.57,and 3.57±6.75 in treatment group and 9.1±3.36,4.89±7.58pre-and-post experimentally.There was obvious difference in CPR decrease in intra-block contrast(P＜0.05).The efficacy of psoriasis of blood-heat type and blood-deficiency was 84.5%and 91.2%in treatment group, obviously higher than those in control group,which were 84.5%and 68.6%. Through follow-up after one year,the recurrent rate in treatment group was obviously lower than that in control group(1.0%in experiment group while 39.3%in control group,P＜0.05).It was found that treatment of Xian ling xiao yin capsule along with routine treatment has better efficiency than that in control group,which provided clinical basis for treatment mechanism.Conclusion1.The effect of Xian ling xiao yin capsule coordinated with routine therapy is much better than that of simple conventional therapy.The mechanism may have something to do with the level of IL-6 and CRP.2.The one year recurrence rate,the treatment group is significant lower than the control group,show that Xian ling xiao yin capsule can promote the curative effect of psoriasis vulgaris with precise effectiveness and no toxic side effects. |
