Clinical Observation Of Jiedu Tongxuan Decoction Medicated Bath In The Treatment Of Psoriasis Vulgaris

Background:Psoriasis vulgaris is a common chronic recurrent disease in clinical practice.It often suffers from lifelong illness,worsens the disease,or even turns into multiple pustules,involving joints or the whole body.Epidemiological surveys show that the number of patients with psoriasis in the country is increasing year by year.Multi-party clinical studies suggest that TCM treatment has a definite effect in the long-term treatment of psoriasis,helps to improve safety,prolong the remission period,and is highly tolerated by patients.Among them,the Xuanfu theory fills the microscopic connotation of TCM theory,and also provides another theoretical basis for the treatment of psoriasis.The external treatment of TCM is also attracting attention because of its "simple and cheap" characteristics.After many years of clinical diagnosis and treatment of psoriasis,the tutor concluded that the medicinal bath prescription of JieDuTongXuan Decoction was effective in the early clinical application,but it lacked the support of experimental data,and it needs further research and demonstration for promotion.Objective:To observe the clinical efficacy and safety of JieDuTongXuan Decoction in the treatment of quiescent psoriasis(blood stasis syndrome).Methods:84 patients with psoriasis meeting the criteria for inclusion in this subject were screened and randomly included into the negative control group,positive control group,and treatment group,with 28 cases in each group.Three groups of patients were given basic treatments such as LiangXueXiaoYu Decoction.The treatment group was given JieDuTongXuan Decoction and calcipotriol ointment for external use;the control group was given calcipotriol ointment for external use.Observe and calculate PASI,pruritus degree score and DLQI before treatment,2 weeks after treatment,4 weeks after treatment,8 weeks after treatment.And during the treatment period,an online follow-up was conducted to record whether there were any adverse reactions.The SPSS26.0 statistical software was used to compare and analyze the data changes of the skin lesion single symptom score,PASI score,pruritus score and DLQI score before and after treatment,and to discuss the clinical efficacy of the treatment methods used in this study on static psoriasis.Also make a comment on its safety.Results:2 cases fell off,and 82 patients completed the test.There are 27 cases in the negative control group,28 cases in the positive control group,and 27 cases in the treatment group.There were no statistically significant differences in gender,age,and course of disease between the three groups of patients(P>0.05).After 8 weeks of treatment,the individual symptom scores,PASI scores,pruritus scores,and DLQI scores of the three groups of patients were significantly improved compared with those before treatment(P<0.05).The difference between each two groups was obtained:The erythema,scale,infiltration,PASI score,pruritus index and DLQI score of treatment group were better than the others(P<0.05).Efficacy judgment:the total effective rate of the negative control group,the positive control group and the treatment group were 62.96%,82.14%,and 92.59%.and the total effective rate between three groups was statistically significant(P<0.05).During the study period,2 patients in the positive control group and 1 patient in the negative control group developed mild gastric bloating with nausea,no acid reflux and vomiting after oral administration of the decoction.Considering the high concentration of granular preparations and other reasons,the patients were instructed to take the medicine after meals.The above symptoms did not recur,Did not affect the entire clinical study.Patients in the treatment group showed no discomfort during the medicated bath.The hematuria routine and specific indexes related to liver and kidney function of the patients before and after treatment were all within the normal reference range.Conclusion:JieDuTongXuan Decoction combined with external calcipotriol ointment in the treatment of static psoriasis vulgaris can significantly improve the individual symptom score,PASI score,pruritus score,and DLQI score of the patient's skin lesions,and the effect is better than simple in the calcipotriol ointment group for external use,there was no obvious adverse reaction.
