Object:To study the general characteristics, syndromes characteristic, syndromes typing of HT with hyperlipidemia and preliminary clarify the law of TCM syndromes.Method:I adopted the questionnaire and physical examination for 190 patients from outpatient and ward of General Department, Guang’anmen Hospital. The main survey included gender, age, dietary preferences, dizziness, headache, and other signs, such as blood pressure, heart rate, lip color, and electrocardiogram, cardiac ultrasound examination. All data were filled in Excel document to set up a database, then were analyzed via SPSS 17.0 statistical software.Results:1. General informationGender age information:Among the 190 patients,98 are female (51.58%),92 are male (48.42%). The oldest is 80 years old, and the youngest is 26,with an average age of 62.79±10.36 years old. The average age of female is 65.66±9.18,while the average age of male is 60.62±11.29 years old.Hypertension disease course, classification, risk stratification:Among the 190 patients, the courses of hypertension vary from 1 to 48 years, and the average disease course is 16.36±9.46 years. As for blood pressure classification,72 (37.90%) are in level 2,86 (45.26%) are in level 3. As for the risk stratification,72 (37.90%) are with high risk,61 (32.10%) are with very high risk. It can be seen that most patients with hypertension combined hyperlipidemia have long duration of high blood pressure, blood pressure levels and risk stratification are high, and the ratio of very high risk patients augments as the increase of blood pressure level.2. TCM syndrome, syndrome type distribution Signs and symptoms distribution of hypertension combined hyperlipidemiaIn this study, the common symptoms of hypertension combined hyperlipidemia are: dizziness, fatigue, insomnia, dry mouth, forgetfumess, bosom frowsty, headache, heart palpitations, tinnitus, etc; Common signs are:oral dark purple, scaly dry skin, dark eye orbital, palm dark red, lower extremity edema, etc. Most are normal tongue, followed by fat big tongue; Tongue are commonly dark red, red, teeth marks, suggillation, dark purple; The common coating of tongue are white greasy, thin white, yellow greasy, scanty fur; Pulse commonly are compound, and the common pulse elements are wiry, slip, sink, string. TCM syndrome type distribution of hypertension combined hyperlipidemiaThis study we collected and recorded the symptoms and signs of 190 patients with hypertension combined hyperlipidemia, then according to the principle of modern statistics, combined with clinical practice, transformed these screened signs and symptoms into 0,1 variable, and through cluster analysis, principal component analysis and rank and inspection, combined with relevant diagnostic criteria and the tutor’s experience, I think five TCM syndrome types are most common in this group, and in this study the distribution of TCM syndrome types are blood-stasis obstruction syndrome> liver-wind phlegm turbidity syndrome> liver-kidney yin deficiency syndrome> hot and humid resistance> spleen kidney yang deficiency syndrome.3. Researches based on TCM syndrome types of hypertension combined hyperlipidemiaIn this study, the types certificate no statistical difference in gender distribution, the distribution in different age groups was statistically significant, The liver-wind phlegm syndrome and hot and humid resistance syndrome occur in low, middle age groups, blood-stasis obstruction syndrome and liver-kidney Yin deficiency syndrome occur in medium and high age groups, while the spleen-kidney Yang deficiency syndrome mainly happens on the high age group. The distribution of liver-kidney yin deficiency syndrome in smoking group was obviously higher than that of without smoking history group; The distribution of hot and humid resistance syndrome in the history of drinking group was obviously higher than that of without history of alcohol group, while the distribution of blood-stasis obstruction syndrome in without history of drinking group was obviously higher than that of group has a history of alcohol consumption. Blood pressure levels, hot and humid resistance syndrome and liver-wind phlegm turbidity syndrome are mainly distributed in level 1, level 2 groups, liver-kidney yin deficiency syndrome are distributed in level 2, level 3 group, spleen-kidney yang deficiency syndrome and blood-stasis obstruction syndrome are mainly distributed in 3 groups. Hypertension risk stratification, hot and humid resistance syndrome distributes in middle-risk group, liver-wind phlegm turbidity syndrome distributes in middle-risk group and high-risk group, liver-kidney yin deficiency syndrome, blood-stasis obstruction syndrome, spleen-kidney yang deficiency syndrome distribute in high-risk group, very high risk group. Hypertension course, the distribution of hot and humid resistance syndrome and liver-wind phlegm turbidity syndrome in the course of 10 or less years group are higher than course> 10 years, while the distribution of blood-stasis obstruction syndrome, liver-kidney yin deficiency syndrome and spleen-kidney yang deficiency syndrome in the course of the disease> 10 years group are higher than the course of 10 or less years group. Weight types, hot and humid resistance syndrome and liver-wind phlegm turbidity syndrome are mainly distributed in overweight and obese, spleen-kidney yang deficiency syndrome are mainly distributed in overweight, blood-stasis obstruction syndrome are mainly distributed in overweight and normal weight, liver-kidney yin deficiency syndrome are mainly distributed in weight, low weight group and normal group. Conclusion:Hypertension combined hyperlipidemia patients have an older average age, but in recent years younger tendency shows up. They usually has a long history of high blood pressure, They are most in blood pressure level 2 or 3. As for the hypertension risk stratification, most patients are in very high risk or high risk. The common symptoms are dizziness, fatigue, insomnia, dry mouth, forgetfulness, bosom frowsty, headache, heart palpitations, tinnitus, et al; The main TCM syndromes types are blood-stasis obstruction syndrome, liver-wind phlegm turbidity syndrome, liver-kidney yin deficiency syndrome, hot and humid resistance, spleen kidney yang deficiency syndrome. The distribution of TCM syndromes types are related with age, history of smoking, history of drinking, hypertension duration, blood pressure levels, risk stratification, body mass index, et al. Blood-stasis obstruction syndrome and liver-wind phlegm turbidity syndrome are the most common types, besides as the main syndromes, the blood stasis syndrome and phlegm turbidity also widely exist in other syndrome types as accompanied symptoms, therefore in the treatment of hypertension with hyperlipidemia, phlegm dispersing and activating blood should raise concern.
