| Objective:To observe the clinical effect of Huahong Capsule with antibiotics on the acute pelvic inflammatory disease (syndrome of damp-heat stasis).Method:84 patients with acute pelvic inflammatory disease (syndrome of damp-heat stasis), collected at the emergency department of West China Second University Hospital from April 2009 to October 2009 were randomly divided into antibiotics group and Huahong group.The antibiotics group was treated with two antibiotics (Program 1:Levofloxacin 500mg,ivgtt,qd;and tinidazole 400mg,ivgtt,bid. Program 2:piperacillin-sulbactam 3g, ivgtt,bid;and azithromycin 0.5g,po,qd).The Huahong group was also given Huahong Capsule 0.75g,po,tid, apart from the same drug administration as the antibiotics group. All the course of treatment was seven days and all patients were followed up for 3 months. The clinical symptoms, signs, blood routine, pelvic inflammationmass average diameter and depth of pelvic fluid were observed. Cost-effectiveness was analyzed on effectiveness. Datas were analysized with SPSS17.0 statistical software.Result:1. Totally 84 cases were enrolled with 3 cases rejectio, and randomly divided into the antibiotics group and the Huahong group with 41 and 40 cases separately. The basic information and conditions of patients between the two groups(antibiotics group, Huahong group) have no differences, P>0.05,such as age (32.73,SD8.78; 31.65,SD8.70 years), courses of disease (2.90,SD2.62;2.60,SD2.30days), tempera-ture(38.13,SD0.75;38.19,SD0.60℃), Patients with neutrophils percentage (N)≥90% have (6,8)cases,80%≤N<90% have (20,16)cases, and 70%0.05. The average inflammation diameter of pelvic mass after treatment for (3.92, SD 2.51cm)was significantly smaller than the antibiotics group with in the antibiotics group(2.09, SD 2.03cm), P<0.05. 3.Evalucation in TCM,, Markedly effective rates of the antibiotics group, and the Huahong group were 70.7% and 87.5%,there is no difference,P>0.05. However, the main syndrome integral of Huahong group after treatment (score=3.65,SD 2.75)were significantly lower than the antibiotics group(6.10,SD 1.29),P<0.001. The cure rates of syndrome of the antibiotics group and the Huahong group were as follows:abdominal pain (68.3%,97.5%),abnormal vaginal discharge (33.3%,65.0%), lumbar sacral pain (56.1%,77.5%), abdominal distension (42.5%,65.8%), chest torpid intake(70.6%,96.7%),mouth dryness like dringking(42.5%,72.5%). The results showed that the Huahong group was more effective than the antibiotics group, P<0.05.4.Follow-up result:The onset of pelvic pain in the following 3 months in the antibiotics group and the Huahong group were respectively 33.3% and 11.8%,P <0.05.5.By cost-effectiveness analysis, the cost and effectiveeness (C/E) in the four programs (Huahong Capsule with antibiotics program 1,Huahong Capsule with antibiotics program 2,simple antibiotics program 1,simple antibiotics program 2) were 14.56,21.35,17.66,24.34 yuan. The C/E of the medication program 1 of Hua-hong Capsule and antibiotics program 1 is the lowest. Increasing the effect of a unit just needs 4.06 yuan more expensive, compared with the simple application of antibiotics program 1.Conclusions:1. For acute pelvic inflammatory disease, the combination of Huahong Capsule with antibiotics can obviously relieve the clinical symptoms, including abdominal and rlumbar sacral pain or tingling, abdominal distension, abnormal vaginal discharge, compared with the use of antibiotics alone.Also, it can reduce the recurrence of pelvic pain.2.Huahong Capsule with Levofloxacin and tinidazole is the best treetment program in this clinical observation.
