Objective To investigate the prevalence rate of metabolic syndrome and its various components, and to determine its influencing factors in the middle aged and elderly people from a community in Hefei city. To evaluate residents'quality of life and to explore the influence of metabolic syndrome on the middle aged and elderly people's quality of life.Methods The cluster sampling method was used to select a community in Hefei city. The residents older than 40 years old in the community were included in this study. Participants'demographic characteristics, health-related behaviors, health status, family medical history, dietary behaviors and dietary intake were obtained. Blood pressure, height, weight, waist circumference were measured. Laboratory measures of fast plasma glucose (FPG), total cholesterol (TC), TG, HDL-C, LDL-C, VLDL-C and serum uric acid were obtained. The whole population was 721, in which 416 persons were male and 305 persons were female. The CDS criteria of MS was used to diagnose the MS patients. Usual dietary intake was assessed by using a semi-quantitative food-frequency questionnaire (FFQ). The residents'quality of life, which was evaluated by WHOQOL-BREF, was compared MS patients with normal people. The comparison of residents'the prevalence of MS by gender and age group was searched by usingχ2 test. The comparison of residents'quality of life between MS patient and non-MS patient by gender, age group and educational level was found by using independent-samples t test. The analysis of multivariable logistic was to investigate the influencing factors of metabolic syndrome. The multiple linear regression was conducted to determine the association of quality of life and MS.Results This study investigated 721 persons, in which 416 persons were male, the mean age was (60.8±10.8) years old, 305 women, the mean age was (59.5±9.7) years old. There was no significant difference between men and women in age (t=1.680, P>0.05). In this study, 88 people were confirmed to MS. The overall prevalence was 12.21%. There was also no significant difference between men(12.74%) and women(11.48%)(χ2=0.263, P>0.05). In the middle aged and elderly population of this community, the most combinations of MS were hyperglycaemia, hypertension and dyslipidemia, of which had 25 persons (31.8%), while the rare combinations of MS were overweight or obesity, hyperglycaemia and dyslipidemia, of which had 6 persons (6.8%). The MS risk components were divided the number as 0, 1, 2, 3, 4, and the number of persons were 255, 244, 134, 73, 15, respectively, accounting for 35.4%, 33.8%, 18.6%, 10.1% and 2.1%. The middle aged people's prevalence of hyperglycaemia and hypertension were lower than the elderly's, while higher than the elderly of dyslipidemia (P<0.05). The prevalence of MS and its each risk component (overweight or obesity, hyperglycemia, hypertension and dyslipidemia) were 12.2%, 23.2%, 16.6%, 40.9% and 29.0%. The obesity prevalence of women was lower than men, the difference was significant (χ2 =6.848, P=0.009). There was no significant difference of the prevalence of hyperglycemia, hypertension and dyslipidemia between men and women (P>0.05). To evaluate the relationship between the prevalence of MS and risk factors, many factors were analyzed. It was demonstrated that education level, LDL-C, VLDL-C, waist circumference, nap habits, family history of diabetes, fatty liver, coronary heart disease were associated with MS. With the waist circumference, education level, LDL-C and VLDL-C increasing, the risk of people suffering from MS also increased. The risk of the people that had family history of diabetes and nap habits suffering from MS was more dangerous than the people without family history of diabetes and nap habits. The risk of the people that had fatty liver and coronary heart disease suffering from MS was also more dangerous than the people without fatty liver and coronary heart disease.Among the middle aged and elderly population, the score of physical, psychological, social relations and the environment are (63.1±12.4), (68.2±10.7), (63.1±11.8) and (64.7±10.3) points for people with MS, while (66.9±12.2), (69.8±10.7), (65.2±11.1) and(65.2±11.0) points for people without. As to male, there was significant difference between people with and without MS in physical score, while as to female, there was not in each score. As to the elderly, there was significant difference between people with and without MS in physical and social relations'score, while as to the middle aged, there was not in each score. As to the middle school and junior secondary schooling, there was significant difference between people with and without MS in each score, while as to high school or above, there was not. After adjusting age, gender and educational level, the quality of life of MS patients in the field of physiology score was lower than the non-MS patients, the difference was significant.Conclusion In the middle aged and elderly people, the prevalence rate of MS was very high. The most common combinations of MS were hyperglycaemia, hypertension and dyslipidemia. It was demonstrated that educational level, LDL-C, VLDL-C, waist circumference, nap habits, fatty liver, coronary heart disease were associated with an increased risk of MS. The MS seemed to have connection with heredity in the middle aged and elderly people. The MS had a certain influence to quality of life, especially for male, elderly and poor education people with MS. Therefore, it's very important to control the development of waist circumference, LDL-C and VLDL-C to prevent the MS and improve people's quality of life.
