Objective: To explore the cognition,attitude and the effecting factors about patients'right of informed consent among doctors,nurses,patients and their relatives. To compare the differences on cognition and attitude of doctors and nurses,patients and their relatives,medical staff group and patient group.Motheds: A total of 80 doctors, 160 nurses,320 patients and 320 relatives in medical, surgical and gynaecological wards who came from 8 hospitals in Changsha and Xiangtan were sampled and asked to complete a self-desiged questionnaires.Results:1. The rates of correct cognition on content of informed consent were 66.8%~98.7% among doctors,nurses,patients and their relatives. The rates of correct answer on the purpose of informed consent were 91.5% and 67.6% differently. There was significantly different between medical staff group and patient group (P=0.000). The rates of correct cognition about the ownership of informed consent were 26.0% and 28.0% differently. The rates of correct answer about the statutory signatory on informed consent document were 37.2% and 23.4% differently.There were different between medical staff group and patient group ,or between doctors and nurses(P=0.000).There wasn't different between doctors and nurses,or between patients and their relatives,or between medical staff group and patient group by comparing the total score of informed consent knowledge(P>0.05).2. There were 42% respondents who answered that medical staff should inform patients with all disease information.The respondents who thought all information about disease shouldn't inform the patients were less than 5%.There were significantly different between doctors and nurses,or between medical staff group and patient group (P<0.01).3. There were more than 87% respondents who thought medical staff should inform the patienst or their relatives with the information related to operation, diagnosis and treatment.But regarding the actual situation of medical fault,the reapondents were 89.9% and 62.3% differently, who considered medical staff should inform patients or their relatives truthfully. There was significantly different (P<0.01).4. There were 87.9% and 70.4% respondents differently who thought informed consent documents being signed was necessary when doing non-risk operation.There was significantly different between medical staff group and patient group (P<0.01).72% respondents considered that informed consent documents should be signed when implementing general examination and treatment.5. 61.9% medical respondents thought that operative informed consent documents should be signed by both patients and their relatives.But 40.7% patients and their relatives considered that it should be signed by patient's relatives. There were different attitudes to the most suitable signatory between medical staff group and patient group(P<0.01).Conclusions:1. Doctors,nurses,patients and their relatives have understoot the content of informed consent clearly,but known about the ownership of informed consent vaguely.The cognitive status on the purpose of informed consent is better medical staff group than patient group.2. On informing patients the information about disease, doctors,nurses and patients' relatives didn't worry about the adverse consequences as informing cancer patients and critically ill patients with information their disease.Patients and their relatives required the information of diagnosis,treatment and the medical fault very strongly.The medical staff were very willing to inform patients or their relative with diagnosis and treatment information,but not willing to inform the information about medical fault.3. Medical staff and patients held the views that informed consent document should be signed when doing non-risk operation and implementing general examination and treatment.4. Patients' relatives are an important signatory on the operative informed consent document.It is an acceptable way that operation informed consent document signed by both patients and their ralatives.
