Objectives: (1) To estimate the prevalence of HIV infection among former paid blood donors in Shandong province, and to analyze high risk factors. (2) To evaluate the epidemic situation of HIV/AIDS in Shandong province. (3) To investiagte the life condition of these donors who are living with HIV/AIDS. (4) To provide scientific evidence for carrying out the policy of "Four Free and One Care". Methods: (1) A cross-sectional study was conducted to invesgate the epidemic situation of HIV/AIDS among former paid blood donors in Shandong province. (2) A through retrospective cohort to study the status among HIV positive in three China Aids Responses(CARE). (3) A follow-up study was carried out to make clear the situation and life level of pelple living with HIV /AIDS in Shandong province. Results: (1) The scale,distribution,character of former paid donors(FPD) in Shandong province: There were 14826 FPD in Shandong province, most of them live in the middle and west area of Shandong province, 53.3% of FPD were in Taian, Heze, Jingning, 47.7% of them are male, 49.8% of them are female, 75.4% of them are farmers, The age of FPD is from 18 to 85, 67.9% of their age are 31-50 years old, 19% of them had history of selling plasma, and many of them sold blood in neighbor provinces. (2) The one-way analysis of HIV infective situation among FPD: There are 208 HIV positive among 14321 former paid blood donors in Shandong province up to Oct 2005, and the HIV positive rate among FPD was 1.5%. Most of them live in Heze, Taian, Qingdao. The HIV positive rate is higher in males than females among FPD (X~2=35.46, P<0.01), The HIV positive rate is significant different in different ages (X~2=98.88, P<0.01), different beginning donation time (x~2=13.04, P<0.01), different donating methods(x~2=500.66, P<0.01), and different donating place (X~2=220.16, P<0.0). (3) The survival analysis of people living with HIV/AIDS among FPD: 28.1% HIV infectors were FPD in Shandong province from 1995 to 2005, 50.5% of them are AIDS, possess 51.7% of people living with HIV/AIDS in Shandong province, 41 cases were died, possess 32.3% of dead cases in Shandong province.
