Significance Studies Of Defensins Concentration,Lymphocyte Subtypes,ASO In Discovery And Therapeutic Evaluation Of Occult HIV/AIDS Patients

Objective:To explore the significance of the clinical indicators included defensins,lymphocytes subtypes,blood routine examination,AST,ALP,GGT,Cr,BUN,UA and ASO in the discovery of occult HIV/AIDS patients,and in efficacy evaluation of AIDS treatment.Methods:From August 2015 to August 2017,103 HIV/AIDS patients without hepatitis virus infection(named HIV/AIDS group)and 103 healthy controls(named control group)were collected randomly from Xixi hospital,Hangzhou,Zhejiang.SPSS 20.0 software were used to analysis the significance of the values including blood routine examination by automatic hematology analyzer,liver and renal function test by automatic biochemistry analyze,lymphocyte count by FCS,and HNP1,HBD2 and BLC-1/CXCL13 by ELISA in AIDS patients and their controls.Two independent samples t test was used for the measurement data conforming to normal distribution.non-parametric test was used for the measurement data conforming to non-normal distribution,and Chi square test was performed for analyzing the statistic significance of the abnormal rates of the items.Results:(1)For blood routine examination,the abnormal rates of white blood cell count(WBC),red blood cell count(RBC),hemoglobin(Hb)and Ret%were12.62%,47.57%,31.07%and 11.65%in HIV/AIDS group,which were significantly higher than control group(P<0.01).However,there was no statistical significance between the abnormal rate of PLT in HIV/AIDS group(5.83%)and control group(4.85%)(P>0.05).The comparison of mean values showed statistically significant differences in WBC,RBC and PLT between the HIV/AIDS group and control group(P<0.05),but no statistical significances on the mean values of Hb and Ret%were found between them(P>0.05).(2)For liver test index,the abnormality rates of AST,ALP and GGT in HIV/AIDS patients with antiviral treatment were 9.71%,30.10%and 24.27%,respectively.All of them were significantly higher than their corresponding control group(P<0.01).The abnormal rate of ALT(13.59%)in the treatment patients was not statistically higher than its control(P>0.05).But the differences of the mean values of these four indexes were significantly higher than its corresponding control(P<0.01).(3)For renal function index,the abnormal rate of CR in patients with antiviral therapy was 17.48%,which was significantly higher than that in the control group(P<0.05).The abnormal rate of detection of BUN and UA was 1.94%and 15.53%in the treated patients,respectively.There were no statistical difference compared with its corresponding the control group(P>0.05).The differences of mean values between the two groups was statistically significant(P<0.001).(4)For rheumatoid disease-related tests,the abnormal rate of ASO detection in antiviral treatment patients was 31.07%,which was higher than that in the control group(2.91%)(P<0.001).The abnormal rate of RF detection was 8.74%in the treated patients and 4.85%in the control group.There was no significant difference between them(P>0.05).(5)For immunological indicators,the abnormal rates of CD4+,CD8+,CD19and NK in patients with antiviral treatment were 13.59%,30.1%,8.74%and 12.62%,respectively.They are all significantly higher than that in the control group(P<0.05).In addition to CD4,the differences of the mean values in other three immunological indexes were statistically higher than those in corresponding control(P<0.05).(6)For HNP1,HBD2 and BLC-1/CXCL13 tests,they were all significantly higher than control groups(P<0.05).Conclusion:(1)the decreasing values of the blood routine test in patients with HIV/AIDS are closely related to the development of AIDS illness and mean also the change of immune function in AIDS patients.It can be used as an important supplement of the CD4+T lymphocyte count in clinical work.(2)the abnormal value of the liver function test post antiviral drugs treatment in patients with normal baseline,is likely to be associated with the adverse reactions.More close detection and the clinical manifestations is a good way to improve the patients medication compliance.(3)the damage of renal function in HIV/AIDS patients is mainly related to the poor management of HIV infection,indicating high level of diagnosis and treatment is required to improve the therapeutic effect.(4)The positive rate of ASO in HIV/AIDS patients is high in some extent,and more attention should be paid to the prevention of G~+streptococcal infection during antiviral treatment.(5)the immune indicators should be combined with the detection results of biochemical indexes as a comprehensive evaluation of treatment efficacy.Dynamic measurement of the patient's immune indicators is of great significance to the control of HIV infection and is one of the important goals of the evaluation and treatment of the disease.
