Analysis Of Status Quo Of Cesarean Section And Its Influences On Mothers And Infants

Cesarean section is an important and operative means to treat high-risk pregnancy. In recent years, the cesarean rate has been rise ,especially the selective operation rate rise rapidly,with the fetal custodial technology progress, anesthetic and antibiotic application advancement and many social factors. the cesarean rate rising has some positive actions to solve pregnancy complications, to decrease the mother and fetal death rate in the perinatal stage, however the cost in hospitalization and the operative complications increase corresponsively. If we continue to raise the rate blindly to some extent, There is a increasing in the relative risk of the maternal mortality also. At present, we have many problem waiting for us to solve. for example, the reason of the cesarean rate rising, the changing of cesarean, when and how to termination of pregnancy.This article retrospectively analyzsed data on all patients delivered in our hospital in 1990,1996,2000,2003.There are 5459 deliveries and among them there are 2151 cesarean deliveries: 934emergency cesarean and 1217 selective cesarean. We investigate. the most important thought factors to proceed Cesarean section with questionmair in the persons without operation reasons and requesting strongly. The result makes clear: (1) cesarean rate and selective cesarean rate evelated markly during the following decade. (2) cephalopelvic factor has a decrease tendence year by year in Cesarean section, society factor become the principle reason especially in selective Cesarean section. (3) the persons which require to proceed the Cesarean section are rising, but the persons which are advanced age and Primiparity are reducing. The most thought factor involve ①worrying not Vaginal delivery and afraiding pain ② worrying the neonate intellence influenced. ③worrying the mother and fetal safety because of fetal macrosomia. (4) There are no differences at 1 minut Apgar score among the none in labor,incubation stage,,active stage,the second stage of labor emergency cesarean.so for this reason, we suggest trial labor should be sufficient to reduce unnecessary cesarean.(5) There are significant differences among different pregnant weeks selective cesarean. The number of the neonate whose birth weight between 2500—3999gm is creasing with creasing of pregnant week.(6)New mode,extraperitoneal and lower segmental cesarean's average days in hospital are about 7days, average cost are about 3700 yuan, there are no differences among them .But the new mode cesarean showed significantly shorter in the time of operative period compared with lower segmental and extraperitoneal type.
