Study On The Application Of "Dredging Therapy" Thought In Spleen And Stomach Disease

[Objective]To tidy up and analyze the ancient and modern literatures,summarize the historical origins of the thought of “Dredging Therapy”,and sum up its application in the field of both Traditional Chinese Medicine and Western Medicine,especially in spleen and stomach diseases.Then verify the practicality and guidance of the thought by clinical study.[Methods and Contents]1.To explore the origins of the thought of “Dredging Therapy” in depth,and sum up its application in spleen and stomach diseases through the analysis of ancient and modern literatures and theories.2.Randomized and controlled clinical trial was adopted in this research.150 patients were randomly divided into 3 groups:Herbs group(50 cases),Acupuncture group(50 cases)and Herbs+Acupuncture group(50 cases),which were respectively treated with “Chaihu WenDan Decoction(CWD)” per os,acupuncture therapy which main of “Dragon-Tiger Fighting(DTF)” acupuncture method on“Ganshu(BL18)” acupiont,and “Chaihu WenDan Decoction” per os combined with acupuncture therapy which main of “Dragon-Tiger Fighting” acupuncture method on“Ganshu” acupiont(CWD+mainly DTF on BL18).All of the cases were treated for 8weeks,and they were respectively followed up in next 2 months.The RDQ scores and the syndrome(symptoms)of stagnation of liver qi and spleen deficiency scores of cases were evaluated and recorded at the time of: before treatment,during treatment(once a week for 4 weeks,and fortnightly for the next 4 weeks)and after the end of treatment(on the forth week and the eighth week).Then the clinical effects,the time when the treatment came into effect and recurernce rate,etc.among three groups were compared and evaluated.[Results]The research was consisted of two parts: theoretical studies on the application ofthe thought of “Dredging Therapy” in spleen and stomach diseases,and clinical trial on Gastroesophageal Reflux Disease(syndrome of stagnation of liver qi and spleen deficiency)which was treated with “Chaihu WenDan Decoction” per os and acupuncture therapy which main of “Dragon-Tiger Fighting” acupuncture method on“Ganshu” acupiont.Part 1: Theoretical studies on the application of the thought of “ Dredging Therapy” in spleen and stomach diseases.1.The thought of “Dredging Therapy” was mainly used in the treatment of obstruent diseases which referred to the respiratory system,circulatory and nervous system,digestive system,endocrine and urinary system,reproductive system,immune system and locomotor system,etc.of Western Medicine.2.Three parts which were viscera adjustment,qi-blood regulation,main and collateral channels clear were included in the application of thought of “Dredging Therapy” in TCM.A variety of methods of TCM which included of lung-qi dispersion,diarrhea,yang-qi tonification,qi promotion and circulation of blood invigoration,etc.were related.3.The physiological activities and pathological changes of spleen and stomach were the foundation of the application of thought of “Dredging Therapy”.And the therapies which were other zang-organs and fu-organs treatment by regulating spleen and stomach or spleen and stomach adjustment by treating other zang-organs and fu-organs were widely used in clinical practice due to “characteristics-generation and restriction” of zang-fu organs.4.“The spleen and stomach are the hub of the movement of qi,that regulation leads to health while obstruction leads to sickness.” was one of Li Pei's academic thoughts.The spleen and stomach disease were cured by Professor Li were always under the guidance of the thought of “Dredging Therapy” in the clinical practice.And the thought was fully manifested on his empirical formula named “Chaihu WenDan Decoction” which was used in the treatment of GERD.Part 2: Clinical trial on Gastroesophageal Reflux Disease(syndrome of stagnation of liver qi and spleen deficiency)which was treated with “Chaihu WenDan Decoction ” per os and acupuncture therapy which main of“Dragon-Tiger Fighting” acupuncture method on “Ganshu” acupiont.1.Improvement of integral of integrated index(symptoms of GERD): The patients who were treated with CWD+mainly DTF on BL18 got the better effects and shorter time when the treatment came into effect than who were treat with mainly DTF on BL18 alone.Compared with CWD,CWD+mainly DTF on BL18 worked more effective in reducing the severity of the attack,but had the similar effects in the reduction of the attack frequency of symptomatic attacks and the time when the treatment came into effect.2.Improvement of integral of specific index(symptoms of GERD): CWD+mainly DTF on BL18 worked better in relieving regurgitation of sourness,regurgitation of foods,heartburn and retrosternal pain than mainly DTF on BL18 alone.On the side of reducing the attack frequency of symptoms: CWD+mainly DTF on BL18 had the better effects of relieving regurgitation of sourness than CWD alone,but they had similar effects in relieving regurgitation of foods,heartburn and retrosternal pain.And CWD worked better than mainly DTF on BL18 in these four symptoms above.On the side of reducing the severity of symptoms: CWD worked better than mainly DTF on BL18 in relieving regurgitation of sourness,regurgitation of foods and heartburn,but got the similar effects in relieving retrosternal pain.CWD alone and CWD+mainly DTF on BL18 also had the similar effects in all of the above four symptoms.Furthermore,both of the CWD and CWD+mainly DTF on BL18 had the shorter time when the treatment came into effect than mainly DTF on BL18 alone.3.Improvement of integral of integrated index(syndromes of stagnation of liver qi and spleen deficiency): The patients who were treated with CWD+mainly DTF on BL18 got the better effects than who were treat with mainly DTF on BL18 alone or CWD alone.CWD and mainly DTF on BL18 had the similar effects.All of these methods of treatment got great effects,but CWD and CWD+mainly DTF on BL18 took the shorter time when the treatment came into effect.4.Improvement of integral of specific index(syndromes of stagnation of liver qi and spleen deficiency): CWD+mainly DTF on BL18 got the better effects of reducing the attack frequency of abdominal pain with desire of defecation and reliving symptoms of loose stool,bowel flatus,abdominal distension,reduced food intake than mainly DTF on BL18 alone.And CWD+mainly DTF on BL18 also got the better effects of reducing the attack frequency of reliving symptoms of reduced food intake than CWD alone.Referred to the effects of reliving desire of a deep sigh,depression or irritability,epigastric distention and pain,CWD+mainly DTF on BL18 worked better than CWD alone.And compared with mainly DTF on BL18 alone,the combined therapy also worked better in reducing the attack frequency of epigastric distention and pain.Both of CWD and mainly DTF on BL18 had the similar effects in the improvement of desire of a deep sigh,loose stool,epigastric distention and pain,abdominal distension,reduced food intake,and reducing the attack frequency of depression or irritability,bowel flatus,abdominal pain with desire of defecation.But for reducing the attack degree of the pain with desire of defecation,there were no significant difference among of the three methods.About the time when the treatment came into effect,CWD+mainly DTF on BL18 worked the best,mainly DTF on BL18 got the shorter time than CWD alone in the improvement of desire of a deep sigh,depression or irritability and the attack degree of epigastric distention and pain.But CWD came into effect earlier than mainly DTF on BL18 in the improvement of loose stool,bowel flatus,abdominal pain with desire of defecation,abdominal distention,reduced food intake and the attack frequency of epigastric distention and pain.5.Evaluation of the overall curative effects(symptoms of GERD): The effective rate of CWD+mainly DTF on BL18 was significantly higher than mainly DTF on BL18 alone,and similar to CWD.The treatment effects of CWD were better than mainly DTF on BL18 alone.6.Evaluation of the overall curative effects(syndromes of stagnation of liver qi and spleen deficiency):The effective rate of CWD+mainly DTF on BL18 was significantly higher than mainly DTF on BL18 or CWD alone.The treatment effects of both of the CWD and mainly DTF on BL18 alone were similar.7.Recurrent rate: CWD>mainly DTF on BL18>CWD+mainly DTF on BL18.[Conclusion]1.The treatment of spleen and stomach disease which is guided by the thought of“Dredging Therapy” has the solid theoretical basis,and it can also be effective in clinical medical practice.2.The treatment methods of “Chaihu WenDan Decoction” per os and acupuncture therapy which main of “Dragon-Tiger Fighting” acupuncture method on“Ganshu” acupiont which are guided by the thought of “Dredging Therapy” have remarkable curative effect on GERD with the syndrome of stagnation of liver qi and spleen deficiency.3.“Chaihu WenDan Decoction” per os works better in reliving main of the GERD symptoms and most part of the symptoms with the syndrome of stagnation of liver qi and spleen deficiency,and which gets shorter time when the treatment comes into effect,but gets higher recurrent rate.4.Acupuncture therapy which main of “Dragon-Tiger Fighting” acupuncture method on “Ganshu” acupiont works better only in reliving small part of symptoms with the syndrome of stagnation of liver qi and spleen deficiency,and which gets longer time when the treatment comes into effect,but gets lower recurrent rate than decoction per os.5.To combine the internal therapy and the external therapy of TCM,more efficient,shorter time of coming effect and lower recurrent rate can be gotten.
