| Purpose:To encourage early detection and early treatment in patients with frozen shoulder.Frozen shoulder is referred to as periarthritis of the shoulder,and is called by the chinese medical practitioners as "Leaky Shoulder Wind","Shoulder Wind","Shoulder Pain","Freezing Shoulder" or "Shoulder Depression" and other different titles.It belongs to the shoulder in the category of "disadvantages" in Chinese medicine.The middle-aged around the age of 50 are more commonly affected,so they are also called fifty shoulders.The etiology of periarthritis of shoulder is due to frail blood and blood deficiency,liver and kidney function gradually decline,lack of qi and blood,poor health and blood circulation,insufficient nutrition of muscles and recurrence of cold and dampness.Tendon injuries resulting in blood stagnation,blocked veins,meridian barrier caused by shoulder pain,limited shoulder activities.Frozen shoulder is limited by shoulder pain and limited shoulder range of motion.Pain is mainly aggravated by movement and relieved at rest.It is often impossible to sleep on the affected side at night and the pain often wakes the patient up in the middle of their sleep.This has a serious impact on the patient's daily life and work.Therefore,the main treatment goal of shoulder periarthritis pain is to solve the pain and cure shoulder function disorder.The purpose of the research project is to compare the clinical effectiveness of three-needle acupuncture plus blood cupping versus cupping and three-needle acupuncture in the treatment of frozen shoulder.The purpose of the research project is also to discuss the clinical efficacy of three-needle acupuncture and blood-letting cupping in the treatment of frozen shoulder.This will provide a more reliable method of treating patients with painful shoulder periarthritis.Methods:100 patients with periarthritis of the shoulder were recruited from December 2016 to December 2017.They were divided into treatment group and control group according to the order of first visit.Each group has 50 cases.The main point:Three needles of the shoulder(Jianyu?jianliao?jianqian)were identified.Patients were told to lie down or sit down exposing the affected side of the shoulder.Routine disinfection was performed.Needles used are 0.30mm× 50mm Han Medicine A one-time needle.The direction of the acupuncture points toward the shoulder joint,the depth of the needle varies according to the patient's specific situation,usually I-2 inch,leaving the needle in for 30 minutes.After needling,the doctor's hand follow the patient's shoulder to find tender points of the shoulder joints in all directions.According to the severity of patient's conditions,3-5 tender points were used for a normal individual whereas 2-3 tender points were used for patients with weakness.Strict disinfection with 75%alcohol sterilized cotton balls,pricking with a 23G number(0.6X40mm)once.Blood sampling needles,rapid pricking,choose different sizes of glass cupping and cover the pricked site.Leave on for 5?10min,can pull out about 1?5ml of blood stasis.After capping,strict cleaning and disinfection of local area and cup.Throughout treatment,there is a local illuminated natural energy lamp shining on the affected needle.After the treatment is completed,the physician teaches the patient to do simple exercises such as wall climbing,body rear handle,elbow hand,and post-extension touch and spine exercises to strengthen shoulder joint.Stretching the atrophic muscles can help to release shoulder adhesions and expand the range of activities of the ipsilateral shoulder.To facilitate the maximum restoration of the active function of ipsilateral shoulder joint,ask the patient to exercise 3-5 times a day,about 20 minutes each time.The acupuncture points in the control group were treated with the same acupuncture points as the treatment group.Acupuncture and blood-letting cupping equipment:The two groups use the same type of needle for acupuncture.Chinese medicine brand disposable acupuncture needles,quality meets the requirements,specifications are 0.30mmx50mm,sterilized cotton balls,alcohol 75%,glass cupping or gas cans,bloodletting using 28G(diameter 0.37mm)disposable blood collection needles,blood collection pens,natural energy lamp,paper towel,medical special recycler.The course of treatment of the two groups were the same,every other day for 10 sessions as one course.Efficacy and index changes were evaluated after one course of treatment.The patients with shoulder periarthritis were evaluated for the activities of daily living scale(ADL),pain score,blood loss from puncture and other aspects.All data were analyzed by SPSS 17.0 software.The count data were analyzed by X2 test,and the grade data were analyzed by Ricit.analysis.Results:1.The treatment group and the control group were statistically processed.There was no significant difference in the shoulder pain,shoulder range,and ADL score between the two groups before treatment(P>0.05).The two groups are comparable.There was significant difference between the two groups after treatment(P<0.05).2.There was a significant difference between the cure rate of the treatment group and the cure rate of the control group.Cure rate for treatment group is 100%whereas cure rate for control group was 86%(P<0.05).3.In terms of clinical treatment efficacy,the treatment group has a better result as compared to the control group in terms of pain score,range of movement and activities of daily living.Conclusions:1.Both group has shown to be effective to relieve pain and improve function of periarthritis of the shoulder.The treatment group is more effective as compared to the control group.2.Statistically,the treatment group is simple and effective,and it is the quickest and most direct way to improve the pain around the shoulder joint as compared to the control group.This method has quick results,short course of treatment,low cost,and no toxic side effects on the human body.It is one of the natural therapies advocated nowadays.It is easy for patients to accept and it is worthy of extensive clinical application and promotion. |
