ObjectiveTo observe the therapeutic effect of proximal needling combining blood-letting puncture and cupping for treating superior gluteal cutaneous nerve entrapment syndrome.MethodsRandomly divide 60 cases of superior gluteal cutaneous nerve entrapment syndrome into 30 observation group and 30 control group respectively.The observation group was given treatments of blood-lett i ng puncture and cuppi ng,proximal needling and conventional acupuncture.Procedure of blood-letting puncture and cupping:after routine disinfection,prick at the midpoint of iliac crest and the tendon-like induration or pressure point in the l ower part to make it bleed.5 minutes after local retaining cupping,take them off and disinfect again.Procedure of proximal needling:prick the acupuncture needle at the tendon-like induration or pressure point with a depth of 0.5-1 inch,use the method of twirling,lifting and thrusting manipulation to achieve needling sensation,then prick another one at the point which is 1 inch beside the first point with an angle of 45 degree which towards to the direction of tendon-like induration or pressure point,and as well using the twirling,lifting and thrusting manipulation method,retain the needle after acquiring the needling sensation.Procedure of conventional acupucture:prick vertically at Shenshu,Dachangshu and Weizhong with a depth of 0.8-1.3 inch.Retain the needle for 30 minutes after requiring needling sensation under the method of twirling,lifting and thrusting needling manipulation.Perform every other day,three times a week.A treatment course last for 2 weeks and patient need to be given 2 courses.The control group was given conventional acupuncture wi th the same acupoint-picking,procedure and course as the observation group.To eval uate the therapeutic offect of before and after treatmen t i n two groups with evaluation scales of VAS and JOA.Results1.There’ s no statistical significance in the differences showing from the VAS grading in observation group and control group before treatment(P>0.05).Statistical significance exists in the difference comparing in prior-treatment with post-treatment in observation group with VAS evaluation scale(P<0.05).Statistical significance exists in the difference comparing in prior-treatment with post-treatment in control group with VAS evaluation scale(P<0.05).Proximal needling combining blood-letting puncture and cupping as well as conventional acupuncture are both effective on reducing pain of superior gluteal cutaneous nerve entrapment syndrome.Statistical significance exists in the difference comparing with VAS between observation group and control group after treatment(P<0.05),which means that proximal needling combing blood-letting puncture and cupping is more effective on reducing pain of superior gluteal cutaneous nerve entrapment syndrome than conventional acupuncture.2.There’s no statistical significance existing in the difference between posttreatment of observation group grading by JOA evaluation scale and control group(P>0.05)·Sattistical significance exists in the difference that comparing after treatmen with posttreatment in observation group by JOA evaluation scale(P<0.05).Sattistical significance exists in the difference that comparing after treatmen with posttreatment in control group by JOA evaluation scale(P<0.05).Proximal needling combining blood-letting puncture and cupping as well as conventional acupuncture are both effective on improving lumbar function of superior gluteal cutaneous nerve entrapment syndrome.Statistical significance exists in the difference comparing with JOA between observation group and control group after treatment(P<0.05),which means that proximal needling combing blood-letting puncture and cupping is more effective on improving lumbar function of superior gluteal cutaneous nerve entrapment syndrome than conventional acupuncture.3.Of therapeutic effect in observation group,7 cured,20 improved,3 not improved,reaches an effective rate of 90.00%.In control group,2 cured,21 improved,7 not improved,effective rate is 76.67%.Statistical significance exists in the difference comparing between observation group and control group after treatment(P<0.05),which means that proximal needling combining blood-letting puncture and cupping as well as conventional acupuncture are both effective on superior gluteal cutaneous nerve entrapment syndrome,and proximal needling combining blood-letting puncture and cupping is more effective than conventional acupuncture.ConclusionProximal needling combining blood-letting puncture and cupping as well as conventional acupuncture are both effective on reducing pain of superior gluteal cutaneous nerve entrapment syndrome,and the therapeutic effect of proximal needling combing blood-letting puncture and cupping is better than conventional acupuncture.
