Objective The operation of laparoscopic hepatectomy (LH) has been carried out more than20years, which has played a huge boost and achieved good results for liver disease. However, it is a surgery of high technical difficulty. The limit of its development is that the procedure has not yet formed systematically. So we need to further explore and standardize its processes. This study is to explore the application and to summarize the techniques and therapeutic the effects of LH.Methods:Retrospective analysis the data of77cases who underwent laparoscopic hepatectomy (LH) from October2012to December2014. The Clinical data and follow-up results of the40patients who underwent LH was compared with that of70patients who underwent laparotomy liver resection during the same period. Comparisons of the two groups in terms of the operative time, blood loss, incision length, gastrointestinal function recovery time, drainage tube indwelling time, complication rate, postoperative hospital stay, the recurrence rate of tumor and survival rate etc. were performed. Summarize the techniques and standardize the surgical procedures of LH in our department.Results:All of the cases were completed successfully and no perioperative deaths happened. Of all the77cases of LH,45cases are benign lesions,32cases are malignant tumors;37cases are irregular excision,40cases are regular resection. Of all the laparotomy operation,17cases are benign lesions,53cases are cancer. Differences of general characteristic, blood loss, survival rate in1year and2year after operation, recurrence rate of tumor between the two group were not statistically significant (P>0.05). There were significant differences between the two groups in terms of the operative time, incision length, gastrointestinal function recovery time, drainage tube indwelling time, complication rate and postoperative hospital stay (P<0.05). Conclusions:Laparoscopic hepatectomy can be selectively applied to various parts of the liver and all types of lesions. Compared with open liver resection, LH have advantages of more beautiful incision, faster recovery, less postoperative complications. The operative time of LH is longer than that of OH. Differences of general characteristic, blood loss, survival rate in1year and2year after operation, recurrence rate of tumor between the two groups were not statistically significant. We can make it easier to promote by using reasonable and standard surgical processes.
