Objective:Based on computational fluid mechanics(CFD),the feasibility of establishing twodimensional fluid mechanics model in urethral cavity after BPH minimally invasive surgery was discussed,and the flow pattern of urine discharge was preliminarily observed.Methods:Choose our transurethral prostatic thulium laser gouge out except in January after adult male(79 years old,BMI of 25.7 and 78.6 g preoperative prostate,IPP 5 mm,the third postoperative day postoperative urine tube,no urethral stricture)in patients with 1,after informed consent,patients in upright position,probe set perineum place,along the longitudinal diameter of urethra prostate surface gathering micturition ultrasound images of the distance and urinary flow rate graphics.The longitudinal diameter of the neck of the bladder and the anteroposterior diameter of the prostatic urethra were measured using the bultrasound image corresponding to the urine flow rate of Qmax(mm,with 1 decimal point reserved).CAD software was used to conduct 2d mathematical modeling.ICEM CFD software was used to conduct mesh processing on the model.Fluent 19.0 software was used to conduct fluid analysis on the mesh model.The center plane of the prostate was taken as the baseline,and the center of the urethra was taken as the origin to draw a two-dimensional curve of distance and urine vector velocity.Results:1.Successfully drew a two-dimensional hydrodynamic model representing the urethral cavity structure after minimally invasive surgery with BPH,and obtained the velocity cloud diagram and vector diagram of the fluid flow in the prostate urethra;2.In the experimental model,the fluid flow velocity was low in the bladder,accelerated in the prostatic urethra,and high in the anterior urethra.3.In the model,there is an obvious vortex phenomenon at the side wall of the prostatic urethra;4.The two-dimensional graph of distance and urine vector velocity indicates that the vortex disappears when the diameter of prostate urethra shrinks.Conclusion:Based on the computational fluid mechanics technology,the method of establishing the two-dimensional fluid mechanics model in the urethral cavity after BPH minimally invasive surgery is feasible.The mathematical inference of the phenomenon of prostate urethral vortex is expected to be the theoretical basis for the analysis of the difference in urination after BPH minimally invasive surgery and the mechanism of complications.
