Clinical And Experimental Studies On Chronic Renal Failure Of Turbid Stasis And Yang Vacuity Pattern Type Treated With Zhentao Decoction

Chronic renal failure (CRF) were studied Systemically including study on theory, experimental study and clinical study. Study on theory: CRF, is caused by various causes, its major pathomechanism is yang vacuity of spleen and kidney, and linger and stagnation of damp turbidity and static blood. Insufficiency of kidney yang can't warm spleen yang, so causes spleen yang vacuity. Insufficiency of the sourse of transformation caused by spleen failing to move and transform leads to kidney lost fullness, and kidney vacuity and yang debilitation, in the end, spleen and kidney yang vacuity and linger and stagnation of damp turbidity and static blood. The former leads to CRF coming on, and the later is the important factors which can cause deteriorating advance gradually of kidney fuction. We follow the methods of Zhongjing?combining formula, combining two formulas, apllying various methods, warming and supplementing spleen and kidney, freeing the bowels and downbearing turbidity, and dispelling dampness and transforming stasis. Experimental study: To test and verify the curative effect of preventing and treating glomerular sclerosis of CRF with Zhenhe decoction, and to probe into its mechanism. With the methods of serum pharmacology, normal SD rats were poured into Zhentao decoction, Zhenwu decoction, Taohechengqi decoction, Benazepril, and physiological saline of low dose and high dose respectively, erm of contenting medicine of every group was collected and was put in the culture dish having cultured human glomerular mesangial cell in vitro, observing the effects on proliferation of glomerular mesangial cell and extracellular matrix(Col LV~ FN~ LN).Zhentao decoction of low and high dose could significantly inhibit the proliferation of glomerular mesangial cell ,fibronetin (FN), laminin(LN), and type LV collagen of ECM(Col IV). Benazepril had any inhibiting effect, but its inhibiting effect was not significant comparing with Zhentao decoction, and it couldn抰 inhibit the proliferation of FN of ECM. The mechanism probabaly is as follows, by inhibiting the proliferation of glomerular mesangial cell, Zhentao decoction decrease the producing of ECM and increase degradation, because abnormal enviroment was alleviated, the abnormal activation and the proliferation of mesangial cell were mitigated, in the end, Zhentao decoction prevent and treat glomerular sclerosis by various ways. Clinical study: To test and verify clinical curative effect of Zhentao decoction treating CRF, and to observe its security. We select the CRF patients being up to the type of spleen and kidney yang vacuity and stagnation of damp turbidity and static blood, and divide into treat group and control group at random according to degree of patient's conditions, basic diseases, and similar ages. Patients of control group took orally Benazepril and were used synthetic therapy, Patients of treat group took orally Zhentao decoction were used synthetic therapy statitic analyse was carried on after treated 60 days.32 cases were treated in two groups respectively Treat group is superior to control group in some aspects(P(O.Ol ~. PKO.05), such as alleviating kidney function and main clinical symptoms, decreasing curative effect intergral of TCM pattern, increasing the content of total protein, albumin, and hemoglobin in serm, decreasing P 2-MG,urine protein, rectifying disorder of dielectric ,and having effects on hemorrheology and so on.
