Case Management In The Community Rehabilitation Of Persons With Mental Disorders Application Research

In recent years,with the development of China’s health undertakings,the Party and the state have paid more and more attention to the rehabilitation of the disabled.In 2021,The State Council issued the Notice on Issuing the Implementation Plan for Rehabilitation Services for the Disabled during the 14 th Five-Year Plan period,which proposed to improve the rehabilitation guarantee policy for the disabled and deepen community rehabilitation for the disabled.Under the circumstance that the number of people with mental disorders may continue to rise,the current service mode of "community rehabilitation based +institutional rehabilitation" in China is difficult to meet the rehabilitation needs of people with mental disorders and their families.It is of great practical significance and professional value to explore the model of community rehabilitation of people with mental disorders.M town of Chongqing is located in the two urban junction,jurisdiction radiation range,scattered,local 68 of mental disorders in 2 years of mental health social work service rehabilitation effect,combined with the local actual situation and mental disorders own system resources to carry out effective community rehabilitation services for local mental disorders to achieve rehabilitation is of great significance.Research on the town of mental disorder rehabilitation demand and the plight of mental health social work research,with five mental disorders for case management service object,according to the needs of the service object and related information design case management scheme,with social ecological system theory,energy theory as guidance,improve mental disorders social adaptability as the goal,use company,listening,energy,empowerment,such as social work service skills,from the service object personal,family,community,social level service,alleviate service object psychological barriers,promote the service object personal ability,improve the service object social support network,etc.Later,the service effect was evaluated according to the interview and observation data,and the problems and practical experience of case management service in the community rehabilitation of people with mental disorders were summarized.The study found that the case management can realize the individual level of psychological problems,enhance their life ability,restore social functions such as social interaction and learning ability,and carrying out family activities,groups and community activities can build the construction of mental disorders medium support network.Finally,combining advocate propaganda work for mental disorders to create a good social rehabilitation atmosphere,case management intervention helps to promote mental disorders to achieve rehabilitation systematicly,comprehensively and constantly,improve the quality and level of community rehabilitation.
