Research On Case Management Of Schizophrenic Patients In Community Rehabilitation

With the expansion of the number of schizophrenic groups,in order to build China’s mental health cause and reduce the negative impact of increasingly prominent mental health problems on the economy,society and the public,China has continuously issued a series of policies and regulations to guide the construction of service delivery system macroscopically,so as to ensure that schizophrenic patients returning to community rehabilitation can enjoy professional services equally.Social work was the main power of basic services.Facing the trend of diversified and complicated needs of clients,how to coordinate the resources of multi-professional teams,such as Civil Affairs Bureau,Subdistrict Office,Mental Health Center,public security department,community staff and other professional teams to accurately meet the physical,psychological,material,development,education and other demands of the patients with schizophrenia and their families during community rehabilitation.there is still room for improvement in many aspects to improve the quality and effectiveness of case management services.Starting from the community dimension,this study was drawn from the community rehabilitation social work service project of mental disorders in S Street,District X,Chongqing,which the author actually participated in.Through the mental protection system,we conducted on-the-spot interviews and telephone follow-up of schizophrenic patients in the community during rehabilitation in S street,and collected the basic data related to the service object,service field and service process in detail.To understand the current situation of psychosocial adaptation of patients with schizophrenia,and to deeply evaluate the plight and needs of patients and families during community rehabilitation,and to observe and analyze the work flow and actual effectiveness of case managers using case management methods to provide rehabilitation services for patients with schizophrenia.Based on this,tthrough empirical research on S Street,it is found that there are four limitations in the case management work of Chinese local community rehabilitation patients with schizophrenia,as follows: the development of multidisciplinary team rehabilitation service resources and links are blocked;The service coordination ability of case managers needs to be improved;Lack of general workflow and template guidance for case management;It is difficult to balance cost control and efficiency improvement.In order to explore the improvement space of case management methods for community rehabilitation schizophrenia patients,countermeasures and corresponding suggestions were proposed from four perspectives:service resources,service skills,service templates,and service plans,as follows:expanding the matching degree between patients’ families and rehabilitation resources;Promote the cultivation of professional abilities of case managers;Refine and innovate case management guidelines;Revise scientific considerations for service planning.This study aims to guide the improvement of case management methods for patients with schizophrenia in community rehabilitation based on consideration of the time,cost,and quality of community rehabilitation projects,enhance the standardization and effectiveness of case management,and then reflect on the national mental health social work service industry,improve the quality and effectiveness of mental health social work services,so that patients with schizophrenia can better recover,resolve difficulties,and return to society in the community..
