Study On "Five-elements Linkage In Community" Home-community Elderly Care Service Model

Nowadays,the problem of aging population in our country is becoming more and more prominent,and the contradiction between supply and demand of elderly care services is constantly emerging.The traditional elderly care model takes the professional elderly care institutions and families as a core,which has been unable to meet the basic needs of the whole society for elderly care services.Especially under the background of active aging,people gradually regard the aging process as a positive and energetic process,and advocates the thoughts of "health","security" and "participation",which gives a higher demand for elderly care services.It is imperative to promote a supply-side reform of elderly care services to build a scientific,efficient and multi-participation integrated home-community elderly care service system.The "Five-elements Linkage in Community" governance model was found by Hubei Province on the basis of summarizing the experience of the "Three-elements Linkage in Community" governance model during the epidemic,through strengthening the leading role of social work,further integrating community volunteer service organizations and public welfare and charity resources.A model for multiple subjects to participate in grassroots governance is an effective improvement and supplement to the " Three-elements Linkage in Community " governance model,which has theoretical and practical significance for further mobilizing social resources and optimizing the grass-roots social governance structure.The research shows that the home-community elderly care service resources has been integrated through " Five-elements Linkage in Community " governance model and the existing home-community elderly care service system has been improved,which has obvious advantages in improving service professionalism,broadening service supply and improving service quality.This study uses the participatory observation method to carry out a more in-depth field study on the community home-based care service project characterized by "Five-elements Linkage in Community" in W community of Wuhan in the form of case study,and comprehensively analyzes the present situation of home-community elderly care services of mode,effectiveness and practical difficulties in W community,and puts forward countermeasures and suggestions for improving the“Five-elements Linkage in Community” Home-community Elderly Care Service Model in response to the requirements of active aging.
