| As China’s population continues to age at an accelerating pace,the burden of chronic diseases is consistently on the rise,leading to an increasing preference for home-based care among the elderly population.In this context,it is crucial to prioritize health management for community seniors with chronic illnesses.However,China’s chronic disease management faces challenges such as imbalanced resource allocation,insufficient health management awareness,and limited capacity.Addressing these issues requires exploring domestic and international experiences to identify effective health management strategies for community elderly individuals with chronic conditions.Social work,as an evolving profession,has the potential to consolidate various social resources,strengthen social support systems,and facilitate self-development among individuals or groups.Consequently,it plays a vital role in advancing health management systems for community-based elderly individuals with chronic diseases.Based on symbolic interaction theory,social support theory,and empowerment theory,the research focuses on the development of community-based elderly health management systems under the objective of healthy aging.It employs professional social work to participate in and coordinate the improvement of community elderly health management services.The study uses questionnaire surveys,interviews,and participant observation to investigate the mechanisms of social work services in managing the health of community elderly individuals with chronic diseases.The research examines the present status of community elderly individuals with chronic diseases in relation to self-care,social support,and health management,identifying and exploring the underlying causes of existing issues.To address the current limitations of the health management system for community elderly individuals with chronic diseases,the study employs social work theories and techniques to implement practical intervention work.Group activities provide scientific health management guidance for community-based elderly individuals with cardiovascular diseases,cultivating a proactive self-management mentality,building long-lasting communication support networks from diverse perspectives such as individuals,communities,and healthcare providers,broadening the approaches elderly patients employ to handle health problems,and boosting their health management capabilities.Lastly,the study offers recommendations for social work intervention in the health management of community elderly patients,aiming to foster the construction and growth of health management systems for community-based elderly patients,address real-world challenges in elderly health management,and lend support to the step-by-step achievement of healthy aging objectives. |
