The party’s "14th Five-Year Plan" National Planning for the Development of the Elderly Cause and the Elderly Service System requires the establishment and improvement of a comprehensive, inclusive and diversified elderly care service system, and continues to meet the multi-level and high-quality healthy elderly care needs of the elderly. In the period of rapid social development and family function transformation, the daily care of the semi-disabled elderly is gradually socialized. At present, the domestic research problems for the semi-disabled elderly are still relatively macroscopic and broad, and the research on the specific problems of the semi-disabled elderly is still relatively lacking, among which there are fewer studies on the social needs of the semi-disabled elderly. The study took the social work service(pilot)project of G City Elderly Care Institutions as an opportunity to take the semi-disabled elderly in G City T Elderly Care Institutions as the research object.Under the guidance of social support theory and empowerment theory, the basic information of the semi-disabled elderly in institutions, the status quo of interpersonal communication and other data were collected through questionnaire survey, participatory observation and unstructured interview, and the problems existing in interpersonal communication of the semi-disabled elderly in nursing institutions were excavated and the cause analysis was carried out.Through data collation,it was found that the semi-disabled elderly in the nursing institution showed passive social interaction, insufficient enthusiasm, low quality of communication, and estrangement in interpersonal relationships. The analysis of its causes is mainly divided into three levels, the personal level:physical disability affects one’s own emotions, personality disorders lead to narrow social circles, social role changes lead to poor adaptation to life, and cognitive biases caused by cultural values. At the family level: broken family structure and lack of family interaction. At the level of elderly care institutions: weak institutional support, unreasonable living arrangements for the elderly, and unreasonable daily work arrangements. Then, according to the analysis of problems and causes, the group work plan was formulated, and through conditional screening, adhering to the principle of voluntary and most needed for the semi-disabled elderly,8 semi-disabled elderly people in institutions were selected as group members to implement group activities.Through the design of six themed activities, including acceptance, interpersonal and communication, listening and praise, the group helps the elderly enhance their interpersonal communication skills and achieve the effect of empowerment. In addition, through the intervention of nursing homes and families,a supportive social environment is built to consolidate and maintain the effectiveness of the group, and a good interpersonal interaction platform is built for the semi-disabled elderly.Based on social support theory and empowerment theory, this paper assists the semi-disabled elderly to rely on the support network in the group to promote their active empowerment and understand and master interpersonal skills. In addition,by linking volunteers, coordinating agency management, and promoting family interaction, social workers can broaden the support network for the semi-disabled elderly from outside the group and increase opportunities for interpersonal interaction,so as to achieve the purpose of external promotion of empowerment. In the group evaluation link, through the evaluation results of group member evaluation, worker evaluation, combined with measurement scale and satisfaction evaluation form,it was shown that the group had a significant effect in interpersonal communication disorders of the semi-disabled elderly in nursing institutions, and achieved the group goal of improving the interpersonal communication disorders of the semi-disabled elderly and improving the interpersonal skills of group members. Finally, the effectiveness of the group’s intervention is summarized, and the group goals of improving interpersonal communication skills, improving interpersonal communication disorders, rebuilding social support networks, assisting in restoring social functions, and improving social adaptability for the semi-disabled elderly in nursing institutions are achieved through group work methods. The disadvantages are the limited depth of intervention in group activities, the limited number of group members, and the difficulty of meeting the individual needs of group members.Through summary and analysis, the following suggestions are put forward at the end of the article on the interpersonal communication barriers existing in semi-disabled elderly care institutions at the government level, the government level should promote the professional development of institutional elderly care services from top to bottom, pension institutions should strengthen their own resource integration capabilities, improve the quality of institutional services, and at the same time families should actively implement the responsibility of the elderly, through the joint cooperation of three different subjects to help the semi-disabled elderly to achieve high-quality elderly care in institutions.
