Research On The Inclusive Model Of Home-based Elderly Care And Health Care In Smart Communities With Social Work Interventio

The intensification of population aging has posed many challenges to China’s traditional elderly care model.Home-based care has gradually become the mainstream mode of care for the elderly.In the early 21 st century,with the development of Internet of things technology,the application of smart communities in home care has gradually been explored,which provides a new way to solve the problem of old-age care.The emergence of issues such as the digital divide among the elderly,weak social support networks,and supply-demand imbalance in home care facilities has made exploring new elderly care models a breakthrough.Based on the social reality of China’s home-based elderly care service industry,this article takes the X community in Chongqing as the research carrier,and selects the all-age elderly group as the research object,this paper studies the problem of social work involvement in the health care for the aged in smart community.By using social support theory and ternary interaction theory,this paper tries to explore the status quo of health care for the aged at home in smart community.By using the methods of literature analysis,field research and statistical investigation,this paper analyzes the existing problems of social work in the process of providing for the aged at home in smart community,and probes into the causes of these problems.In the process of practice,social work services are carried out by means of case study,group work and community work,explore the process of participation in the problems,causes and ways of Breakthrough.The findings are as follows: social work,as the main force in the construction and operation of the old-age service center and the old-age service station,makes use of specialized and large-scale services,and combines the policy and operation mechanism of home-based oldage care in Chongqing,in the process of carrying out intelligent community service,the“center belt stations” home-based care for the aged is developed.After clarifying the relationship between the service center and the service site,the researcher refines the model of health care and inclusion in smart community.To explore the construction and long-term development of the“center belt stations” effectively promoted by social work under the model of health care for the aged,it provides certain facts and theoretical basis for the new social work to intervene in the home-based elderly care service in the smart community.
