Study On Social Work Intervention In Community Home Care Service Under The Background Of Medical And Health Care

With the emergence of diversified old-age services,community home care has become the best choice of old-age care.Under the concept of healthy aging,the government puts forward the elderly care service mode of combining medical care and health care with community home care service,providing the elderly with the integration of medical treatment,rehabilitation and old-age care,which meets the multi-level needs of the elderly.But at this stage the mutiple level endowment service,by the mere force community is not enough,so the need of social work through scientific theory and method to practice of community home endowment service,for the community elderly people to build a social support network and innovative community home endowment service content,to promote the medical,the improvement of the community home endowment service mode.This study takes the home care service of W community in Luoyang City as an example,comprehensively uses the literature research method,questionnaire survey method and semi-structured interview to carry out the research.Through the study of the process of social work intervention in the home care service of W community,the current situation of home care service in W community and the specific needs of the elderly are analyzed.From health care,life care and spiritual comfort and the needs of self growth four aspects to develop community home endowment service plan,specific project implementation process of interpretation and evaluation,to explore the social work role during intervention as well as the existing problems,and puts forward countermeasures of optimizing the social work intervention in community home endowment service proposal,To enrich the practical experience of Chinese social work in community home care service and look into the future development of medical and health care community home care service,and provide new ideas and methods for innovating community home care service mode.
