Objective:Through clinical comparative observation,objective evaluation of the clinical efficacy and safety of acupuncture needle-finger tremor and qi-promoting method in the treatment of chronic pelvic inflammatory disease characterized by abnormal leucorrhea provides a new,effective and safe acupuncture treatment method for clinical treatment of this disease.Method:Seventy-two patients who met the diagnostic criteria and inclusion criteria were randomly divided into treatment group and control group by digital randomization.There were 36 cases in the treatment group(needle-finger tremor and qi-promoting method group)and 36 cases in the control group(conventional acupuncture group).The treatment group was treated by acupuncture at Sanyinjiao and Baihuanshu points with the method of needle-finger tremor and qi-promoting method,and the method of supplementing and reducing the remaining points was used.All points in the control group were treated with the method of calming,reinforcing and reducing.Both groups were treated three times a week,once every 1-2 days,4 weeks as a course of treatment,and observed after 3 courses(acupuncture stopped during menstrual period).After treatment,VAS score,quantitative criteria of local physical signs grading,integral of TCM syndromes and signs,and determination of pelvic effusion depth were used to evaluate the score of patients before and after treatment.All patients will be treated with SPSS25.0 statistical software after treatment.Result:1.Baseline:A total of 72 patients with CPID were included in this study,and were randomly divided into treatment group(36 cases)and control group(36 cases).After treatment,2patients in the treatment group fell off,and 0 patients were removed or terminated.In the control group,3 patients fell off,and 0 patients were removed or terminated.Finally,67 effective cases were completed,and the dropout rate was 13.89%.The number of patients in the two groups was 34 in the treatment group and 33 in the control group.Before treatment,there was no significant difference between the two groups in terms of age and course of disease(P>0.05),and the two groups were comparable at baseline.2.Efficacy evaluation(1)Comparison of VAS score results before and after treatment: within-group comparison: VAS scores of patients in both groups decreased significantly after treatment compared with before,and the difference(P < 0.01)was highly statistically significant,indicating that both groups achieved efficacy in relieving patients’ pain.Inter-group comparison: After treatment,the VAS score of the treatment group was lower than that of the control group(P=0.005<0.05)through the rank sum test of two independent samples,indicating that the treatment group was superior to the control group in terms of pain relief,and the difference was statistically significant.(2)Comparison of quantitative grading scores of TCM symptoms before and after treatment: intra-group comparison: after treatment,TCM symptoms scores were lower in both groups than before treatment(P < 0.01),and the difference was extremely statistically significant.Inter-group comparison: The difference between the two groups in TCM syndrome scores has great statistical significance(Z=-3.213,P=0.001<0.05),which proves that the needle-finger tremor and qi-promoting method is significantly better than the control group in improving TCM syndrome.(3)Comparison of single item scores of sub-band symptoms between the two groups before and after treatment: intra-group comparison: the scores of both the treatment group and the control group were reduced compared with those before treatment(P < 0.01),with significant differences.Between-group comparison: the single item scores of hypospadias in the treated group were significantly lower than those in the control group after treatment(Z=-2.482 P=0.013<0.05),indicating that the improvement of hypospadias symptoms by the needle-finger tremor and qi-promoting method was significantly better than that in the control group.(4)Comparison of the quantitative standard scores of local signs grading between the two groups before and after treatment: intra-group comparison: both the treatment group and the control group played a better efficacy in terms of improvement of local signs of chronic pelvic inflammatory disease after treatment(P < 0.01).Between-group comparison:the difference in scores between the two groups after treatment was statistically significant(Z=-2.192 P=0.028 <0.05),proving that the efficacy of the acupuncture method was significant in terms of local symptom improvement.(5)Comparison of pelvic fluid depth between two groups before and after treatment:intra-group comparison: after analysis by paired sample rank sum test,P < 0.01 for both groups,with an extremely significant statistical difference,proving that both treatment and control group treatment had a significant effect on the reduction of pelvic fluid.Comparison between groups: After the treatment was analyzed by independent sample rank sum test,Z =-0.841,P = 0.400 > 0.05,the difference was not statistically significant.This indicates that there is no significant difference between the needle-finger tremor and qi-promoting method and conventional acupuncture in improving pelvic effusion.(6)Comparison of the total effective rate and comprehensive efficacy between the two groups: in the assessment of the efficacy of Chinese medical symptoms,the effective rate of the treatment group reached 91.18%,and the effective rate of the control group was78.79%;in the assessment of the efficacy of local signs,the effective rate of the treatment group was 88.24%,and the effective rate of the control group was 72.73%;in the comprehensive efficacy,the total effective rate of the treatment group was 91.18%,and the total effective rate of the control group was In terms of overall efficacy,the total effective rate was 91.18% in the treatment group and 78.79% in the control group.The differences in clinical efficacy were statistically significant(P < 0.05).In conclusion,the overall efficacy of the treatment group was better than that of the control group.(7)Safety evaluation: both groups of patients showed no adverse reactions,good general condition and stable vital signs in the records before and after treatment;no stagnant needles or bent needles occurred during the needling process,and no organ organs were stabbed,indicating a level 1 safety and high safety during the treatment.Conclusion:1.Both the needle-finger tremor and qi-promoting method group and the conventional acupuncture method group could reduce the pain,symptoms and signs of patients with chronic pelvic inflammatory disease,and had obvious clinical efficacy.And the clinical efficacy of the needle-finger tremor and qi-promoting method was significantly better than that of the control group.2.Both theneedle-finger tremor and qi-promoting method group and the conventional acupuncture method group could reduce the symptoms of pelvic effusion in chronic pelvic inflammatory disease,but the difference in the degree of improvement between the two treatment modalities was small.3.Both the treatment group and the control group did not experience any adverse reactions or abnormalities of acupuncture throughout the treatment process,indicating that the needle-finger tremor and qi-promoting method is safer in clinical treatment. |
