Objective:To observe the clinical effect of modified Shiquan Dabu decoction on hidden blood loss of senile intertrochanteric fracture in perioperative period,and to provide a new therapeutic idea for reducing hidden blood loss of senile hip fracture after operation with traditional Chinese medicine.Methods:Strictly in accordance with the criteria for inclusion and exclusion,a total of 60 cases of femoral intertrochanteric fracture were 2021 in the Department of Orthopaedics and Traumatology of Yunnan Provincial Hospital of Traditional Chinese medicine(First Affiliated Hospital of Yunnan University of Traditional Chinese medicine)from October to 2023 February,they were randomly divided into control group and Observation Group,30 in each group.The control group was treated with PFNA internal fixation,the patients were treated with anticoagulation(enoxaparin sodium),prevention of infection(cefazolin sodium),analgesia(Lofn codeine sustained-release tablets)and blood transfusion(allogeneic blood transfusion when hemoglobin was less than 80G/L)Observation Group on the basis of the control group,continuous oral 10 days Shiquan da Butang Jiawei decoction(from the first postoperative day).Red blood cell count(RBC),Hematocrit(HCT),Hemoglobin(HB)and D-dimer(DD)were recorded before and 1,3,7 days after operation The VAS pain scores were recorded before operation and on the 1st,3rd,7th and10 th day after operation,and the TCM symptom scores were recorded on the 1st,3rd,7th and 10 th day after operation The total apparent blood loss,total recessive blood loss,total blood loss,allogeneic blood transfusion,blood transfusion rate and the curative effect and complication rate on the 7th day after operation were calculated.All findings were analyzed statistically using SPSS26.0 software.Results:1.Comparison of general data: There were no significant differences in age,sex,height,body mass,BMI,Evans-Jense classification,Operation Time,blood transfusion volume and blood transfusion rate between the two groups(P> 0.05),the results showed that there was no significant difference in case data between the control group(routine treatment)and the observation group(routine treatment + modified Shiquan Dabu decoction).2.Comparison of laboratory indicators:There was no significant difference in RBC,HCT and HB between the two groups before operation and on the 1st and 3rd day after operation(P> 0.05),but on the 7th day after Operation,RBC,HCT in the control group were lower than that in the observation group,there was no significant difference in D-diner before operation and on the 1st,3rd and 7th day after operation(P> 0.05).3.Comparison of blood loss:There was no significant difference in total dominant blood loss between the two groups(P> 0.05),but the total recessive blood loss and total blood loss between the control group(routine treatment)and the observation group(routine treatment+modified Shiquan Dabu decoction)were significantly lower than those in the control group(routine treatment +modified Shiquan Dabu decoction),the difference was statistically significant(P<0.05).4.Comparison of Vas pain scores:There was no significant difference between the two groups in Vas score before operation and on the 1st and 3rd day after operation(P>0.05).There were significant differences in VAS scores between the two groups on the 7th and 10 th day after operation(P< 0.05),the VAS scores of the two groups were decreased before operation and on the 1st,3rd,7th and 10 th day after operation.There were significant differences between the two groups(P< 0.05).5.Comparison of TCM symptom score:TThere was no significant difference between the two groups on the 1st and 3rd day after operation(P>0.05),on the 7th and 10 th day after operation,the scores of TCM symptoms in the observation group(routine treatment +modified Shiquan Dabu decoction)were lower than those in the control group(routine treatment),and the differences were significant(P< 0.05),the integral of the two groups decreased gradually,and there were significant differences between the two groups(P< 0.05).6.Comparison of curative effect of TCM symptoms: The total effective rate was 93.40% in the observation group and 86.7% in the control group(P<0.05).7.Comparison of complication rate: The incidence of complications in the control group(16.67%)was higher than that in the observation group(10.00%).There was no significant difference in the incidence of complications between the two groups(P> 0.05).Conclusion:The modified Shiquan Dabu decoction can obviously reduce the hidden blood loss in peri-operative period of senile intertrochanteric fracture of femur,relieve postoperative pain and improve the symptoms of deficiency of qi and blood after hip operation.
